妊娠期高血壓疾病患者圍生期凝血指標(biāo)及血小板參數(shù)的變化研究

【摘要】 目的：探討妊娠期高血壓疾病患者在圍生期的凝血指標(biāo)和血小板參數(shù)變化情況。方法：納入筆者所在醫(yī)院2014年2月-2018年12月確診為妊娠期高血壓疾病的產(chǎn)婦150例為觀察組，另選取同時期90例健康分娩產(chǎn)婦為對照組，檢測并比較兩組產(chǎn)婦產(chǎn)前、第一產(chǎn)程、第二產(chǎn)程和產(chǎn)后5 min四個時間點(diǎn)的凝血指標(biāo)凝血酶原時間（PT）、部分凝血活酶時間（APTT）、凝血酶時間（TT）和纖維蛋白原（Fib）和血小板參數(shù)血小板計數(shù)（PLT）、血小板平均體積（MPV）、血小板分部寬度（PDW）。結(jié)果：觀察組PT、APTT、TT值均低于對照組，F(xiàn)ib值高于對照組，差異均有統(tǒng)計學(xué)意義（P<0.05）;兩組第二產(chǎn)程PT、APTT、TT均低于其他時間段，F(xiàn)ib值均高于其他時間段，差異均有統(tǒng)計學(xué)意義（P<0.05）。觀察組PLT低于對照組，MPV和PDW均高于對照組，差異均有統(tǒng)計學(xué)意義（P<0.05）;兩組第二產(chǎn)程PLT均低于其他時間段，MPV和PDW均高于其他時間段，差異均有統(tǒng)計學(xué)意義（P<0.05）。結(jié)論：妊娠期高血壓疾病產(chǎn)婦圍生期凝血指標(biāo)與血小板參數(shù)具有明顯變化，應(yīng)根據(jù)不同時段的變化加強(qiáng)干預(yù)，預(yù)防嚴(yán)重并發(fā)癥的發(fā)生。

[Abstract] Objective： To investigate the changes of blood coagulation indexs and platelet parameters in perinatal period in hypertensive patients in pregnancy. Method： A total of 150 parturients with hypertensive disorder complicating pregnancy diagnosed in my hospital from February 2014 to December 2018 were observed as the observation group， in addition， 90 healthy parturients in the same period were selected as the control group. The coagulation index prothrombin time （PT）， partial thrombin time （APTT）， thrombin time （TT） and fibrinogen （Fib） and Platelet parameters Platelet count （PLT）， platelet average volume （MPV）， platelet branch width （PDW） were measured and compared between the two groups before delivery， first labor， second labor and postpartum 5 min. Result： The values of PT， APTT and TT in the observation group were lower than that of the control group， and the value of Fib was higher than that of the control group， the differences were statistically significant （P<0.05）. The second stage of labor PT， APTT and TT of the two groups were lower than the other time periods， and the Fib values were higher than that of other time periods， the differences were statistically significant （P<0.05）. In the observation group， PLT was lower than that of the control group， and both MPV and PDW were higher than that in the control group， the differences were statistically significant （P<0.05）. The PLT of the second stage of labor in the two groups was lower than that in other time periods， MPV and PDW were higher than those in other time periods， the differences were statistically significant （P<0.05）. Conclusion： The perinatal coagulation index and platelet parameters of pregnant women with hypertension have obvious changes. The intervention should be strengthened according to the changes of different time periods to prevent the occurrence of serious complications.
