[摘要]"目的"基于腦小血管病(cerebral"small"vessel"disease,CSVD)影像學(xué)總負(fù)荷構(gòu)建急性腦梗死(acute"cerebral"infarction,ACI)患者住院時間延長列線圖模型,并對其進(jìn)行驗(yàn)證。方法"選取2021年1月至2023年12月湖州學(xué)院附屬南太湖醫(yī)院神經(jīng)內(nèi)科收治的462例ACI患者作為研究對象,按照7∶3原則,將患者分為訓(xùn)練組(n=323)和驗(yàn)證組(n=139)。采用Lasso-Logistic回歸分析ACI患者住院時間延長危險因素,采用受試者操作特征曲線(receiver"operating"characteristic"curves,ROC曲線)預(yù)測效能。結(jié)果"基于訓(xùn)練組數(shù)據(jù),Lasso回歸篩選出4個非零系數(shù)指標(biāo),分別為基線美國國立衛(wèi)生研究院卒中量表(National"Institutes"of"Health"stroke"scale,NIHSS)評分、年齡校正查爾森合并癥指數(shù)(age-adjusted"Charlson"comorbidity"index,"aCCI)評分、中性粒細(xì)胞與淋巴細(xì)胞比值(neutroph"to"lymphocyte"ratio,NLR)、CSVD總負(fù)荷評分。多因素Logistic回歸分析顯示,基線NIHSS評分、aCCI評分、NLR、CSVD總負(fù)荷評分均是ACI患者住院時間延長的獨(dú)立危險因素(Plt;0.05)?;谏鲜?項指標(biāo)構(gòu)建列線圖模型,預(yù)測訓(xùn)練組、驗(yàn)證組患者住院時間延長的ROC曲線下面積為0.812(95%CI:"0.756~0.868)、0.820(95%CI:"0.730~0.909)。結(jié)論"基于CSVD總負(fù)荷評分構(gòu)建的ACI患者住院時間延長列線圖模型具有較好的預(yù)測效能,可作為篩查ACI患者住院時間延長的評估工具。

住院時間是評價醫(yī)院診療水平、醫(yī)療資源利用率的重要監(jiān)測指標(biāo)。研究表明住院時間延長不僅增加急性腦梗死(acute"cerebral"infarction,ACI)患者疾病負(fù)擔(dān),還可增加患者并發(fā)癥發(fā)生率,影響其預(yù)后[1-2]。明確ACI患者住院時間延長危險因素并進(jìn)行早期預(yù)警,對縮短患者住院時間、改善其預(yù)后具有重要的臨床意義。腦小血管?。╟erebral"small"vessel"disease,CSVD)是腦卒中常見發(fā)病原因,約占全部病因的25%[3]。研究表明CSVD總負(fù)荷評分與ACI病情進(jìn)展密切相關(guān)[4-6]。目前,針對ACI研究多集中于發(fā)病機(jī)制、并發(fā)癥及預(yù)后等方面,入院影像學(xué)特征對患者住院時間的影響尚不明確。本研究通過Lasso-Logistic回歸分析ACI患者住院時間延長危險因素,并基于CSVD總負(fù)荷評分構(gòu)建ACI患者住院時間延長列線圖模型,為臨床工作提供參考。
1""對象與方法
1.1""研究對象及資料收集
選取2021年1月至2023年12月湖州學(xué)院附屬南太湖醫(yī)院神經(jīng)內(nèi)科收治的462例ACI患者作為研究對象,其中男293例,女169例,年齡31~88歲,平均(67.35±11.35)歲。按照7∶3原則,將患者分為訓(xùn)練組(n=323)和驗(yàn)證組(n=139)。納入標(biāo)準(zhǔn):①年齡≥18歲;②符合ACI診斷標(biāo)準(zhǔn)[7]且經(jīng)顱腦MRI證實(shí);③首次發(fā)病。排除標(biāo)準(zhǔn):①死亡/中途轉(zhuǎn)院;②臨床資料缺失。通過電子病歷系統(tǒng)收集患者入院臨床資料,包括年齡、性別、體質(zhì)量指數(shù)(body"mass"index,BMI)、發(fā)病至入院時間、基線美國國立衛(wèi)生研究院卒中量表(National"Institutes"of"Health"stroke"scale,NIHSS)評分、吸煙史、飲酒史、醫(yī)保類型、高血壓病史、糖尿病病史、高脂血癥病史、冠心病病史、心房顫動病史、年齡校正查爾森合并癥指數(shù)(age-adjusted"Charlson"comorbidity"index,aCCI)評分、中性粒細(xì)胞與淋巴細(xì)胞比值(neutrophil"to"lymphocyte"ratio,NLR)、血小板計數(shù)(platelet"count,PLT)、血紅蛋白(hemoglobin,Hb)、血糖(glucose,Glu)、同型半胱氨酸(homocysteine,Hcy)、尿酸(uric"acid,UA)、白蛋白(albumin,ALB)、總膽固醇(total"cholesterol,TC)、甘油三酯(triglyceride,TG)、D-二聚體(D-dimer,D-D)、CSVD總負(fù)荷評分。兩組患者的一般資料比較差異均無統(tǒng)計學(xué)意義(Pgt;0.05),見表1。本研究定義ACI患者住院時間≥14d為住院時間延長。本研究經(jīng)湖州學(xué)院附屬南太湖醫(yī)院醫(yī)學(xué)倫理委員會批準(zhǔn)(倫理審批號:HZNTHYY-LL-LC-"2022-07-001)。
1.2""統(tǒng)計學(xué)方法
采用R"4.0.5"統(tǒng)計學(xué)軟件對數(shù)據(jù)進(jìn)行處理分析,符合正態(tài)分布的計量資料以均數(shù)±標(biāo)準(zhǔn)差(")表示,組間比較采用t檢驗(yàn),不符合正態(tài)分布的數(shù)據(jù)以中位數(shù)(四分位數(shù)間距)[M(Q1,Q3)]表示,組間比較采用秩和檢驗(yàn),計數(shù)資料以例數(shù)(百分率)[n(%)]表示,組間比較采用χ2檢驗(yàn)。Plt;0.05為差異有統(tǒng)計學(xué)意義。采用Lasso回歸篩選特征變量和多因素Logistic雙向逐步回歸構(gòu)建ACI住院時間延長預(yù)測模型,受試者操作特征曲線(receiver"operating"characteristic"curves,ROC曲線)分析預(yù)測價值。
2""結(jié)果
2.1""ACI患者住院時間延長多因素Logistic回歸分析
104例ACI患者住院時間延長?;€NIHSS評分、aCCI評分、NLR、CSVD總負(fù)荷評分均是ACI患者住院時間延長的獨(dú)立危險因素(Plt;0.05),見表2。
2.2""列線圖模型構(gòu)建與驗(yàn)證
ACI患者住院時間延長列線圖見圖1。ROC曲線顯示該模型在訓(xùn)練組、驗(yàn)證組中預(yù)測住院時間延長的曲線下面積分別為0.812(95%CI:"0.756~"0.868)、0.820(95%CI:"0.730~0.909),見圖2。
3""討論
近10年,中國的ACI年齡標(biāo)準(zhǔn)化發(fā)病率增長34.7%,住院時間是評估某種疾病診療水平和醫(yī)療質(zhì)量管理的重點(diǎn)指標(biāo)[8-12]。本研究基線NIHSS評分、aCCI評分、NLR、CSVD總負(fù)荷評分均是導(dǎo)致ACI患者住院時間延長的獨(dú)立危險因素。Yuan等[13]研究表明基線NIHSS評分每增加1分,ACI患者溶栓后發(fā)生再出血轉(zhuǎn)化風(fēng)險上升1.29倍。Han等[14]研究表明高基線NIHSS評分增加ACI患者不同階段神經(jīng)功能惡化發(fā)病風(fēng)險。另有Meta分析顯示,基線NIHSS評分也與ACI患者并發(fā)惡性腦水腫獨(dú)立相關(guān)[15]。
基線NIHSS評分與ACI患者住院時間延長獨(dú)立相關(guān),究其原因可能是基線NIHSS評分越高,ACI患者神經(jīng)功能受損越重,導(dǎo)致患者出現(xiàn)各種并發(fā)癥的風(fēng)險越顯著上升,從而延長治療時間。aCCI評分是一種基于年齡、合并癥進(jìn)行量化整合分析的評分系統(tǒng)。研究表明高aCCI評分與腫瘤患者中遠(yuǎn)期預(yù)后獨(dú)立相關(guān)[16]。Chang等[17]研究表明aCCI評分≥4分行雙側(cè)全膝關(guān)節(jié)置換術(shù)患者的術(shù)后30d、90d再入院風(fēng)險分別增加3.37倍和2.31倍。此外,隊列研究表明aCCI評分與中高跌倒風(fēng)險老年人群骨質(zhì)疏松發(fā)病率呈正相關(guān)[18]。ACI患者多為高齡人群,合并基礎(chǔ)疾病比例較高,目前aCCI評分與ACI關(guān)聯(lián)性尚不明確。Meta分析結(jié)果顯示,高NLR與ACI患者靜脈溶栓后出血轉(zhuǎn)化、3個月神經(jīng)功能預(yù)后不良獨(dú)立相關(guān)[19]。云靜等[20]研究表明NLR水平與腦梗死體積呈正相關(guān)。ACI發(fā)病早期,中性粒細(xì)胞可釋放活性氧、炎性物質(zhì)、細(xì)胞因子等破壞患者血-腦脊液屏障,加劇腦組織損傷;患者應(yīng)激后免疫抑制導(dǎo)致淋巴細(xì)胞數(shù)量減少[21]。NLR快速上升,提示ACI患者腦組織損傷越重,治療難度越大。CSVD屬于一種大腦微血管疾病,其影像學(xué)標(biāo)志物可單獨(dú)存在或多種并存。研究表明相比單一影像學(xué)標(biāo)志物,多種標(biāo)志物聯(lián)合使用對觀察事件的預(yù)測價值更優(yōu)[22-23]。
近年來,臨床逐漸使用CSVD總負(fù)荷評分用于全面評估CSVD影像學(xué)標(biāo)志物的累積效應(yīng)。研究顯示與CSVD總負(fù)荷評分0~1分相比,CSVD總負(fù)荷評分2~4分的ACI患者發(fā)生梗死灶出血轉(zhuǎn)化、顱內(nèi)出血風(fēng)險上升3.1倍和2.86倍[24]。CSVD總負(fù)荷評分越高,提示ACI患者腦小血管功能受損越重,更易進(jìn)一步損傷血-腦脊液屏障、引起血管內(nèi)皮功能障礙、加劇炎性反應(yīng)等,影響患者神經(jīng)功能恢復(fù),延長治療周期。
本研究存在一定不足:本研究為單中心、回顧性研究,樣本量較少,今后需開展多中心、前瞻性研究進(jìn)一步驗(yàn)證。綜上,基線NIHSS評分、aCCI評分、NLR、CSVD、總負(fù)荷評分是導(dǎo)致ACI患者住院時間延長的危險因素,基于上述4項指標(biāo)構(gòu)建的ACI患者住院時間延長列線圖模型預(yù)測效能較好。
