[摘要]"目的"探討多模態(tài)超聲心動(dòng)圖對(duì)肺動(dòng)脈高壓患者右心功能的診斷效能。方法"選取2022年1月至2023年8月于金華市中心醫(yī)院就診的80例肺動(dòng)脈高壓患者納入觀察組,根據(jù)患者的肺動(dòng)脈收縮壓將其分為輕度組[40~50mmHg(1mmHg=0.133kPa),36例]、中度組(51~70mmHg,20例)和重度組(gt;70mmHg,24例);選取同期健康體檢者80名納入對(duì)照組,所有受試者均進(jìn)行多模態(tài)超聲心動(dòng)圖檢測(cè),對(duì)比其超聲檢查相關(guān)數(shù)據(jù),評(píng)估多模態(tài)超聲心動(dòng)圖對(duì)肺動(dòng)脈高壓患者右室收縮功能的診斷效能。結(jié)果"觀察組患者的右室舒張末期容積(right"ventricular"end"diastolic"volume,RVEDV)、右室收縮末期容積(right"ventricular"end"systolic"volume,RVESV)、右室整體無效做功(right"ventricular"global"waste"work,RVGWW)均顯著高于對(duì)照組,右室每搏輸出量(right"ventricular"stroke"volume,RVSV)、右室射血分?jǐn)?shù)(right"ventrical"ejection"fraction,RVEF)、三尖瓣環(huán)收縮期位移(tricuspid"annular"plane"systolic"excusion,TAPSE)、整體縱向應(yīng)變(global"longitudinal"strain,GLS)、右室整體做功效率(right"ventricular"global"work"efficiency,RVGWE)、右室整體做功指數(shù)(right"ventricular"global"work"index,RVGWI)、右室整體有效做功(right"ventricular"global"constructive"work,RVGCW)均顯著低于對(duì)照組(Plt;0.05)。重度組患者的RVEDV、RVESV及RVGWW均顯著高于中度組、輕度組,RVSV、RVEF、RVGWE、RVGWI、RVGCW均顯著低于中度組、輕度組(Plt;0.05)。TAPSE、GLS、RVSV、RVEF、RVGWI五項(xiàng)聯(lián)合診斷肺動(dòng)脈高壓患者右心功能的曲線下面積為0.911,敏感度為92.23%,特異性為96.45%。結(jié)論"多模態(tài)超聲心動(dòng)圖能有效評(píng)價(jià)肺動(dòng)脈高壓患者的右室收縮功能,值得臨床推廣應(yīng)用。

肺動(dòng)脈高壓的主要特點(diǎn)是肺動(dòng)脈壓力升高[1]。肺動(dòng)脈壓升高可使患者右心結(jié)構(gòu)功能改變,從而導(dǎo)致右心衰竭,甚至死亡。研究表明輕、中度肺動(dòng)脈高壓時(shí),右室功能可代償,但長(zhǎng)時(shí)間和重度肺動(dòng)脈高壓則可引起不可逆的右室損傷[2]。相關(guān)研究證實(shí),當(dāng)肺動(dòng)脈收縮壓≥50mmHg(1mmHg=0.133kPa)時(shí),3年病死率超過90%,主要病因?yàn)榉蝿?dòng)脈高壓引起的急性右心衰竭[3]。因此,早期識(shí)別肺動(dòng)脈高壓導(dǎo)致的右室收縮功能異常尤為重要。本研究通過將無創(chuàng)心肌做功技術(shù)、實(shí)時(shí)三維超聲心動(dòng)圖、M型超聲心動(dòng)圖及二維斑點(diǎn)追蹤技術(shù)各參數(shù)進(jìn)行比較,分析多模態(tài)超聲心動(dòng)圖在評(píng)估肺動(dòng)脈高壓患者右室收縮功能中的應(yīng)用價(jià)值。
1""資料與方法
1.1""一般資料
選取2022年1月至2023年8月于金華市中心醫(yī)院就診的80例肺動(dòng)脈高壓患者納入觀察組。納入標(biāo)準(zhǔn):①符合臨床肺動(dòng)脈高壓的診斷標(biāo)準(zhǔn);②無其他嚴(yán)重的并發(fā)癥。排除標(biāo)準(zhǔn):①與左心疾病相關(guān)的肺動(dòng)脈高壓;②合并心律失?;驀?yán)重并發(fā)癥;③合并先天性心臟??;④超聲圖像質(zhì)量不滿意或不能配合檢查的患者;⑤拒絕超聲檢查的患者。根據(jù)三尖瓣反流速度、下腔靜脈內(nèi)徑及下腔靜脈吸氣塌陷率估測(cè)肺動(dòng)脈收縮壓,再根據(jù)肺動(dòng)脈收縮壓將觀察組患者分為輕度組(40~50mmHg,36例)、中度組(51~70mmHg,20例)和重度組(gt;70mmHg,24例)。隨機(jī)選取同期健康體檢者80名納入對(duì)照組。所有研究對(duì)象均簽署知情同意書,本研究經(jīng)金華市中心醫(yī)院倫理學(xué)委員會(huì)審查批準(zhǔn)[倫理審批號(hào):(研)2024-倫理審查-40]。
1.2""方法
1.2.1""操作方法""受試者左側(cè)臥位,連接心電圖肢體導(dǎo)聯(lián),使用Philips"CVx超聲診斷儀和X5-1探頭,頻率為2.0~5.0MHz,獲取心尖四腔心切面,取樣線平行于右室側(cè)壁三尖瓣環(huán)運(yùn)動(dòng)方向,使用M型超聲心動(dòng)圖測(cè)得三尖瓣環(huán)收縮期位移(tricuspid"annular"plane"systolic"excusion,TAPSE),再使用二維斑點(diǎn)追蹤技術(shù)獲取并分析右室縱向應(yīng)變圖像,測(cè)得整體縱向應(yīng)變(global"longitudinal"strain,GLS)。實(shí)時(shí)三維超聲心動(dòng)圖:3D"Auto"RV軟件自動(dòng)識(shí)別并確認(rèn)位置,描記心內(nèi)膜;在右室基底部、中部、心尖部水平的三個(gè)短軸切面,手動(dòng)調(diào)整收縮末期和舒張末期心內(nèi)膜追蹤線,儀器系統(tǒng)自動(dòng)追蹤、分析并計(jì)算,經(jīng)過對(duì)右室的三維容積和功能的分析,得出相關(guān)參數(shù),即右室舒張末期容積(right"ventricular"end"diastolic"volume,RVEDV)、右室收縮末期容積(right"ventricular"end"systolic"volume,RVESV)、右室每搏輸出量(right"ventricular"stroke"volume,RVSV)、右室射血分?jǐn)?shù)(right"ventrical"ejection"fraction,RVEF)。
心肌做功圖像采集:使用GE"Vivid"E95超聲儀器,M5S探頭,4Vc探頭,于大動(dòng)脈短軸采集肺動(dòng)脈瓣血流頻譜,四腔心切面采集三尖瓣血流頻譜存儲(chǔ)備用,于以右室為主的心尖四腔心、右室流入道切面連續(xù)采集3個(gè)心動(dòng)周期的動(dòng)態(tài)圖像;將原始數(shù)據(jù)圖像拷貝至EchoPAC工作站進(jìn)行圖像分析,在Myocardiac"Work模式下軟件輸出右室心肌做功參數(shù),即右室整體做功指數(shù)(right"ventricular"global"work"index,RVGWI)、右室整體有效做功(right"ventricular"global"constructive"work,RVGCW)、右室整體無效做功(right"ventricular"global"waste"work,RVGWW)、右室整體做功效率(right"ventricular"global"work"efficiency,RVGWE)。
1.2.2""觀察指標(biāo)""①對(duì)比分析兩組研究對(duì)象的M型超聲心動(dòng)圖、二維斑點(diǎn)追蹤技術(shù)、實(shí)時(shí)三維超聲心動(dòng)圖及無創(chuàng)心肌做功技術(shù)參數(shù);②M型超聲心動(dòng)圖、二維斑點(diǎn)追蹤技術(shù)、實(shí)時(shí)三維超聲心動(dòng)圖及無創(chuàng)心肌做功技術(shù)對(duì)肺動(dòng)脈高壓患者右室收縮功能的診斷價(jià)值。
1.3""統(tǒng)計(jì)學(xué)方法
采用SPSS"20.0軟件對(duì)數(shù)據(jù)進(jìn)行統(tǒng)計(jì)分析。計(jì)量資料符合正態(tài)分布的以均數(shù)±標(biāo)準(zhǔn)差()表示,觀察組與對(duì)照組比較采用t檢驗(yàn),多組間比較采用方差分析,觀察組內(nèi)兩兩比較采用LSD法;不符合正態(tài)分布的以中位數(shù)(四分位數(shù)間距)[M(Q1,Q3)]表示,比較采用Wilcoxon秩和檢驗(yàn);計(jì)數(shù)資料以例數(shù)(百分率)[n(%)]表示,比較采用χ2檢驗(yàn)。Plt;0.05為差異有統(tǒng)計(jì)學(xué)意義。
2""結(jié)果
2.1""兩組研究對(duì)象的超聲參數(shù)比較
觀察組患者的RVEDV、RVESV、RVGWW均顯著高于對(duì)照組,RVSV、RVEF、TAPSE、GLS、RVGWE、RVGWI、RVGCW均顯著低于對(duì)照組(Plt;0.05),見表1、表2。
2.2""不同程度肺動(dòng)脈高壓患者無創(chuàng)心肌做功技術(shù)及實(shí)時(shí)三維超聲心動(dòng)圖參數(shù)比較
重度組患者的RVEDV、RVESV及RVGWW均顯著高于中度組、輕度組,RVSV、RVEF、RVGWE、RVGWI、RVGCW均顯著低于中度組、輕度組(Plt;0.05),見表3、表4。
2.3""多模態(tài)超聲心動(dòng)圖對(duì)肺動(dòng)脈高壓患者右心功能的診斷價(jià)值
結(jié)果顯示,TAPSE、GLS、RVSV、RVEF、RVGWI診斷肺動(dòng)脈高壓患者右心功能的曲線下面積(area"under"the"curve,AUC)分別為0.887、0.807、0.760、0.749、0.859,五項(xiàng)聯(lián)合的診斷價(jià)值最高,AUC為0.911,敏感度為92.23%,特異性為96.45%,見表5。
3""討論
肺動(dòng)脈高壓是臨床常見的心血管病癥之一,其主要特征為肺血管阻力異常增加,早期病癥隱匿且缺乏典型表現(xiàn),因此誤診率較高。早期患者以右室代償性增厚、擴(kuò)張為主要表現(xiàn),若未能得到及時(shí)治療,則可能導(dǎo)致右心功能衰退,嚴(yán)重者可致右心衰竭從而導(dǎo)致患者死亡[4-7]。因此,盡早診斷對(duì)肺動(dòng)脈高壓患者的預(yù)后尤為重要。
基于右室解剖結(jié)構(gòu)和形態(tài)學(xué)特點(diǎn),精準(zhǔn)評(píng)估右室功能比較困難,心臟磁共振成像是目前公認(rèn)的評(píng)價(jià)右室功能的無創(chuàng)金標(biāo)準(zhǔn),而在實(shí)際應(yīng)用中因價(jià)格昂貴且操作費(fèi)時(shí)并未成為首選檢查方案。
實(shí)時(shí)三維超聲心動(dòng)圖可實(shí)時(shí)對(duì)心臟的整體形態(tài)及各室壁的運(yùn)動(dòng)情況進(jìn)行重現(xiàn),而后經(jīng)軟件處理直接得出相關(guān)參數(shù),并可多次重復(fù)測(cè)量,具有動(dòng)態(tài)三維成像準(zhǔn)確、實(shí)時(shí)、無創(chuàng)的優(yōu)勢(shì)[8-10]。無創(chuàng)心肌做功技術(shù)作為近年來新出現(xiàn)的一種定量評(píng)估心臟功能的新技術(shù),通過將二維斑點(diǎn)追蹤獲得的應(yīng)變曲線與左室壓力曲線相結(jié)合測(cè)量心肌做功,可解決使用應(yīng)變分析時(shí)存在的負(fù)荷依賴問題[11-12]。已有研究證實(shí)用心肌做功評(píng)價(jià)右室功能的可行性和準(zhǔn)確性[13-16]。心臟磁共振的RVEDV、RVESV、RVSV、RVEF參數(shù)是臨床評(píng)價(jià)右心功能的金標(biāo)準(zhǔn)。本研究發(fā)現(xiàn)伴隨著肺動(dòng)脈高壓患者肺動(dòng)脈收縮壓的升高,RVEDV、RVESV升高,RVEF降低,與既往研究結(jié)果一致[17-19]。無創(chuàng)心肌做功技術(shù)參數(shù)表明重度肺動(dòng)脈高壓患者的RVGWE、RVGWI、RVGCW明顯低于輕度肺動(dòng)脈高壓及健康者,RVGWW明顯高于輕度肺動(dòng)脈高壓及健康者,表明重度肺動(dòng)脈高壓右室整體及流入道功能受到不同程度的損傷。本研究亦發(fā)現(xiàn),與M型超聲心動(dòng)圖、二維斑點(diǎn)追蹤技術(shù)相比,實(shí)時(shí)三維超聲心動(dòng)圖、無創(chuàng)心肌做功技術(shù)對(duì)肺動(dòng)脈高壓患者右心功能診斷價(jià)值較高,與既往研究結(jié)果一致[20-21]。TAPSE是通過局部運(yùn)動(dòng)推測(cè)整體右室收縮功能的指標(biāo),其有角度依賴性,因此具有局限性,右心室GLS僅局限于二維,而實(shí)時(shí)三維超聲心動(dòng)圖可解決角度依賴性且不局限于二維空間。無創(chuàng)心肌做功技術(shù)能定量評(píng)估心肌做功,可解決超聲心動(dòng)圖技術(shù)中存在的角度依賴和負(fù)荷依賴。
綜上所述,多模態(tài)超聲心動(dòng)圖能有效評(píng)價(jià)肺動(dòng)脈高壓患者的右室收縮功能,值得臨床推廣應(yīng)用。
