血清4項指標(biāo)聯(lián)合檢測在結(jié)直腸癌中的臨床意義*

目的 探討血清癌胚抗原(CEA)、糖鏈抗原199(CA199)、糖鏈抗原125(CA125)和糖鏈抗原724(CA724)聯(lián)合檢測在結(jié)直腸癌輔助診斷和臨床參數(shù)的相關(guān)性。方法 回顧性分析結(jié)直腸癌組164例、結(jié)直腸良性病變組56例、健康對照組49例的血清CEA、CA199、CA125和CA724濃度,統(tǒng)計分析4種腫瘤標(biāo)志物在健康對照組、結(jié)直腸良性病變組和結(jié)直腸癌組中的陽性率以及其濃度與結(jié)直腸癌臨床病理資料的相關(guān)性。結(jié)果 結(jié)直腸癌患者血清CEA、CA199、CA125和CA724陽性率明顯高于結(jié)直腸良性病變患者及健康者,差異有統(tǒng)計學(xué)意義(P<0.05),聯(lián)合檢測血清CEA、CA199、CA125和CA724可明顯提高結(jié)直腸癌診斷的敏感度;血清CEA與結(jié)直腸癌局部浸潤程度、淋巴結(jié)轉(zhuǎn)移、遠(yuǎn)處轉(zhuǎn)移和臨床分期相關(guān);血清CA199、CA125和CA724濃度與遠(yuǎn)處轉(zhuǎn)移相關(guān);血清CA724在結(jié)直腸黏液腺癌中明顯高于其他病理類型;血清CA125和CA724與大腸癌解剖部位有關(guān);血清CEA和CA199與癌組織Ki-67表達(dá)水平呈負(fù)相關(guān)。結(jié)論 血清CEA、CA199、CA125和CA724對結(jié)直腸癌臨床輔助診斷有一定價值,4者聯(lián)合檢測可提高結(jié)直腸癌的檢出率;血清CEA、CA199、CA125和CA724與結(jié)直腸癌臨床病理參數(shù)有關(guān),可作為結(jié)直腸癌預(yù)后和預(yù)測標(biāo)志物。

近年來,隨著經(jīng)濟的發(fā)展、生活方式改變和環(huán)境惡化等因素的影響,結(jié)直腸癌在中國發(fā)病率明顯增高[1]。據(jù)統(tǒng)計,2009年結(jié)直腸癌發(fā)病率居于第三位(29.44/105),病死率居于第五位(14.23/105)[2]。因血清腫瘤標(biāo)志物檢測具有低侵襲性和依從性高特點,臨床上常常利用檢測多種血清腫瘤標(biāo)志物用于腫瘤輔助診斷、療效檢測和預(yù)后監(jiān)測。在此,本研究擬回顧性分析常用腫瘤標(biāo)志物癌胚抗原(CEA)、糖鏈抗原199(CA199)、糖鏈抗原125(CA125)和糖鏈抗原724(CA724)在結(jié)直腸癌臨床輔助診斷的價值及其濃度與結(jié)直腸癌臨床病理參數(shù)的相關(guān)性,以期為臨床應(yīng)用提供參考,現(xiàn)將研究結(jié)果報道如下。
1.1 一般資料 選取2011年1月1日至2013年11月1日本院收治的因腹痛、腹瀉、血便、腸鏡示腸癌等入院并檢測血清CEA、CA199、CA125和CA724的患者為研究對象。納入標(biāo)準(zhǔn):(1)術(shù)前抽血檢測CEA、CA199、CA125和CA724腫瘤標(biāo)志物;(2)均經(jīng)腸鏡篩查分析;(3)結(jié)直腸良性疾病和結(jié)直腸癌患者均經(jīng)病理確診。排除標(biāo)準(zhǔn):(1)結(jié)直腸癌患者入院前已給予手術(shù)放化療等治療;(2)結(jié)直腸癌患者未經(jīng)影像學(xué)技術(shù)對肺、肝、膽、脾、腎、膀胱、前列腺及子宮等評估;(3)有腫瘤病史、代謝異常、發(fā)生急性心血管疾病者;(4)肺、肝、膽、脾、腎、膀胱、前列腺及子宮等可見占位性病變者,子宮不明原因增大。共納入結(jié)腸直癌164例,結(jié)直腸腺瘤息肉良性疾病56例,健康對照組49例。記錄患者的性別、年齡、病理診斷結(jié)果,術(shù)前血清CEA、CA199、CA125和CA724檢測值以及病理類型、腫瘤大體類型、組織學(xué)分級、浸潤深度、淋巴結(jié)轉(zhuǎn)移情況及遠(yuǎn)處臟器轉(zhuǎn)移情況、臨床分期等臨床病理資料。
1.2 血清腫瘤標(biāo)志物檢測 所有患者均采用空腹抽取靜脈血,使用全自動化學(xué)發(fā)光免疫測定系統(tǒng)(美國,Abbott)檢測血清中腫瘤標(biāo)志物CEA、CA199、CA125和CA724濃度。CEA、CA199、CA125和CA724正常范圍為分別為小于10 ng/mL、小于37 U/mL、小于35 U/mL、小于6 U/mL。
1.3 統(tǒng)計學(xué)處理 采用SPSS17.0軟件對數(shù)據(jù)進行處理及統(tǒng)計學(xué)分析。經(jīng)統(tǒng)計分析表明本資料呈非正態(tài)分布,因此各個標(biāo)志物濃度采用中位數(shù)(全距)描述,并采用Mann-Whitney 檢驗或Kruskal-Wallis 檢驗對不同疾病組間血清腫瘤標(biāo)志物濃度進行比較分析,利用卡方檢驗對不同疾病組間陽性率進行比較分析。以α=0.05為檢驗水準(zhǔn),P<0.05為差異有統(tǒng)計學(xué)意義。
2.1 血清CEA、CA199、CA125和CA724在健康對照、結(jié)直腸良性疾病和不同分期結(jié)直腸癌中陽性比例 血清CEA、CA199、CA125和CA724在結(jié)直腸癌中濃度明顯高于健康對照組和結(jié)直腸良性疾病組,差異有統(tǒng)計學(xué)意義。見表1。
根據(jù)公司提供的參考范圍,血清CEA、CA199、CA125和CA724在結(jié)直腸癌中的陽性率明顯高于健康對照組和結(jié)直腸良性疾病組,4者聯(lián)合檢測可明顯提高結(jié)直腸癌診斷的陽性率,差異有統(tǒng)計學(xué)意義(P<0.05)。雖4者聯(lián)合檢測可明顯提高Dukes′B、C和D結(jié)直腸癌診斷的陽性率,但對A期結(jié)直腸癌診斷的陽性率仍較低,差異有統(tǒng)計學(xué)意義(P<0.05)。見表2。
2.2 不同結(jié)直腸癌病理參數(shù)血清CEA、CA199、CA125和CA724濃度的比較 通過分析不同結(jié)直腸癌病理參數(shù)血清CEA、CA199、CA125和CA724濃度,發(fā)現(xiàn)結(jié)直腸黏液性癌血清CA724濃度明顯高于其他組織類型;血清CA125在除乙狀結(jié)腸以外的其他結(jié)腸中濃度最高,在直腸中濃度最低;血清CA724濃度在除乙狀結(jié)腸以外的其他結(jié)腸中濃度最高,在乙狀結(jié)腸中濃度最低;血清CEA、CA199、CA125濃度在結(jié)腸癌Dukes′C和D期的濃度明顯高于Dukes′A和B期;血清CEA、CA199濃度在Ki-67≤40%的陽性情況明顯高于Ki-67>40%。見表3。

為了克服單個腫瘤標(biāo)志物診斷敏感性和特異性明顯較低的缺點,常常聯(lián)合檢測多種腫瘤標(biāo)志物[3]。陳創(chuàng)等[4]報道聯(lián)合CEA+f-PSA+CA125+CA242或CEA+CA19-9+CA125+f-PSA可明顯提高結(jié)直腸癌診斷的準(zhǔn)確性。本研究證明CEA、CA199和CA724聯(lián)合檢測可提高其診斷結(jié)直腸癌診斷的準(zhǔn)確性,且三者結(jié)合CA125可進一步提高其在結(jié)直腸癌診斷的敏感性。與其他報道一致,本研究表明血清CEA濃度與結(jié)直腸、局部浸潤、淋巴結(jié)轉(zhuǎn)移、遠(yuǎn)處轉(zhuǎn)移和臨床分期有關(guān);有淋巴結(jié)轉(zhuǎn)移者明顯高于無轉(zhuǎn)移者;有遠(yuǎn)處轉(zhuǎn)移者明顯高于無遠(yuǎn)處轉(zhuǎn)移者[5]。大量研究表明血清CA199、CA125、CA724濃度與結(jié)直腸癌預(yù)后相關(guān),但本研究未發(fā)現(xiàn)與結(jié)直腸癌預(yù)后有關(guān)的血清CA199、CA125、CA724濃度與腫瘤局部浸潤、淋巴結(jié)轉(zhuǎn)移、結(jié)直腸癌遠(yuǎn)處轉(zhuǎn)移相關(guān)[6-7]。這一差異性可能與本研究樣本量不足或樣本選擇性有關(guān)。本研究證實血清腫瘤標(biāo)志物濃度與結(jié)直腸部位和病理類型相關(guān),如CA724在結(jié)直腸黏液性癌血清中濃度最高;血清CA125在除乙狀結(jié)腸以外的其他結(jié)腸中濃度最高,在直腸中濃度最低;血清CA724濃度在除乙狀結(jié)腸以外的其他結(jié)腸中濃度最高,在乙狀結(jié)腸中濃度最低。因此兩者可用于預(yù)測大腸癌的解剖部位。
有關(guān)Ki-67與結(jié)直腸癌預(yù)后關(guān)系的文獻(xiàn)報道并不一致。這一差異性可能與腫瘤細(xì)胞的異質(zhì)性有關(guān),如癌細(xì)胞侵襲組織邊緣Ki-67表達(dá)水平明顯低于腔緣組織,且癌細(xì)胞侵襲組織邊緣組織ki-67表達(dá)水平較低者預(yù)后較差。最近,F(xiàn)luge等[8]報道在結(jié)腸癌Ki-67≥40%者無復(fù)發(fā)生存率高Ki-67<40%者,尤其是術(shù)后經(jīng)放化療進一步處理者Ki-67≥40%者明顯優(yōu)于Ki-67<40%者;但在直腸癌Ki-67的表達(dá)水平與其預(yù)后無關(guān)。同樣在本研究表明Ki-67<40%者血清CEA和CA19-9濃度高于Ki-67≥40%者,基于血清CEA、CA199濃度越高,預(yù)后越差[9]。由此可見Ki-67的預(yù)后價值與其表達(dá)部位、大腸癌解剖部位、治療方案等有關(guān)。
總之,聯(lián)合檢測血清CEA、CA199、CA125、CA724可提高結(jié)直腸癌診斷的敏感性,而且其濃度與結(jié)直腸癌病理參數(shù)相關(guān),可作為結(jié)直腸癌輔助診斷、預(yù)測和預(yù)后標(biāo)志物。

Clinical significance of serum 4 indexes combined detection in colorectal cancer*

Objective To explore the correlation between the combined detection of CEA,CA199,CA125 and CA724 with the assisted diagnosis of colorectal cancer and clinical parameters.Methods The serum levels of CEA,CA199,CA125 and CA724 in 164 patients with colorectal cancer,56 patients with benign colorectal diseases and 49 normal control subjects were performed the retrospective analysis,and furthermore the positive detection rates of these 4 kinds of tumor marker in the normal control,benign colorectal diseases and colorectal cancer and the correlation between the levels of above markers and clinical clinicopathological data of colorectal cancer were statistically analyzed.Results The positive rates of serum CEA,CA199,CA125 and CA724 in the patients with colorectal cancer were significantly higher than those with benign lung diseases and normal controls,the differences were statistically significant (P<0.05);the combined detection of serum CEA,CA199,CA125 and CA724 could obviously increase the sensitivity for diagnosing colorectal cancer;the serum CEA level was related to the tumor local invasion degree,lymph node metastasis,distant metastasis and clinical staging;the serum CA199,CA125 and CA724 levels were related to the distant metastasis of colorectal carcinoma;the serum CA724 level in mucinous adenocarcinoma was higher than that in the other pathological types;the serum CA125 and CA724 levels were related to tumor anatomic location;the serum CEA and CA199 levels were negatively correlated to the Ki-67 expression level of colorectal carcinoma tissue.Conclusion The serum levels of CEA,CA199,CA125 and CA724 has certain value for the clinical assisted diagnosis of colorectal cancer,their combined detection could improve the detection rate of colorectal cancer.The serum levels of CEA,CA199,CA125 and CA724 are associated with the clinicopathological parameters and can serve as the markers of prognosis and prediction of colorectal cancer.
