額葉癲癇；噘嘴發(fā)作

1 病例報告

患者女，23歲，慢性病程，因“反復(fù)發(fā)作性口角下撇15年余”就診,發(fā)作時表現(xiàn)：發(fā)作性口角下撇，伴雙眼瞪大、表情恐懼，持續(xù)時間10～20 s/次，5～6次/d。無四肢抽搐，無咂嘴、咀嚼。查體：神清，精神可。神經(jīng)系統(tǒng)檢查未見陽性定位體征。既往史、家族史無特殊。頭顱MRI：未見異常。發(fā)作間期頭皮腦電圖：可見左側(cè)額極、額、中央?yún)^(qū)棘波、 棘慢波散發(fā)或持續(xù)性發(fā)放；發(fā)作期頭皮腦電(記錄到3次發(fā)作)：清醒期，雙眼睜大，表情恐懼，口角下撇，同期腦電圖表現(xiàn)為左側(cè)額極、額、中央?yún)^(qū)低-中波幅棘波、棘慢波、快波活動發(fā)放→廣泛電壓減低，持續(xù)時間約20 s(圖1、2)。治療方案：奧卡西平0.9 g，1次/早；0.6 g，1次/晚，以及開普蘭0.5 g，2次/d,口服。2017-06隨訪:目前近半年發(fā)作1次,藥物調(diào)整為奧卡西平0.3 g，2次/d,開普蘭0.5 g,2次/d。
