冠狀動(dòng)脈造影及介入治療致造影劑腎病的危險(xiǎn)因素分析

目的觀察不穩(wěn)定型心絞痛患者在冠狀動(dòng)脈造影(coronary angiography,CAG)或非急診經(jīng)皮冠狀動(dòng)脈介入治療(percutaneous coronary intervention,PCI)前后腎功能的變化以及造影劑腎病(contrast-induced nephropathy,CIN)的發(fā)生率,探討CIN的危險(xiǎn)因素。方法不穩(wěn)定型心絞痛患者500例,在手術(shù)前后行水化治療的基礎(chǔ)上,測(cè)定不同時(shí)間胱抑素C(Cystatin C,Cys C)、血清肌酐(serum creatinine,SCr)及腎小球?yàn)V過(guò)率(estimated glomerular filtration rate,eGFR),計(jì)算CIN發(fā)生率并分為CIN組及非CIN組,評(píng)估CIN組手術(shù)前后腎功能變化。比較2組的臨床特點(diǎn),對(duì)其進(jìn)行CIN危險(xiǎn)評(píng)分(Mehran評(píng)分系統(tǒng)),分析CIN發(fā)生的危險(xiǎn)因素。結(jié)果500例入選患者中72例發(fā)生了CIN,發(fā)生率為14.4%。CIN組年齡、入院時(shí)Cys C和SCr水平、造影劑用量、糖尿病比例、高血壓比例、病變嚴(yán)重程度比例、Mehran評(píng)分高于非CIN組,eGFR低于非CIN組,差異均有統(tǒng)計(jì)學(xué)意義(P<0.05)。CIN組手術(shù)后Cys C和SCr水平呈先升高再降低趨勢(shì),eGFR水平呈先降低后升高趨勢(shì),差異均有統(tǒng)計(jì)學(xué)意義(P<0.05)。多因素Logistic回歸分析結(jié)果顯示,入院Cys C水平、入院eGFR水平、糖尿病病史、造影劑用量及Mehran評(píng)分是CIN的危險(xiǎn)因素。結(jié)論即使預(yù)防性應(yīng)用水化治療,CIN發(fā)病率仍然很高。將Cys C及SCr結(jié)合有利于提高CIN的檢出率。造影劑用量、糖尿病病史、基礎(chǔ)腎功能不全及Mehran評(píng)分是CIN的獨(dú)立危險(xiǎn)因素,其中Mehran評(píng)分相關(guān)性最好,可于術(shù)前推廣應(yīng)用。

隨著冠狀動(dòng)脈介入治療技術(shù)的發(fā)展,含碘造影劑的使用愈發(fā)廣泛,造影劑所導(dǎo)致的造影劑腎病(contrast-induced nephropathy,CIN)發(fā)病率逐漸升高,是冠狀動(dòng)脈造影(coronary angiography,CAG)及經(jīng)皮冠狀動(dòng)脈介入治療(percutaneous coronary intervention,PCI)的一個(gè)重要的并發(fā)癥,成為急性腎損傷(acute kidney injury,AKI)的第三大原因,僅次于腎灌注不足和腎毒性藥物引起的AKI,占全部醫(yī)院獲得性腎衰竭的11%[1-2]。CIN發(fā)生急性腎損害的患者,導(dǎo)致早期病死率升高和住院時(shí)間延長(zhǎng),其遠(yuǎn)期腎功能下降的風(fēng)險(xiǎn)亦升高[3-4],增加腎臟及心血管事件的發(fā)生率,增加透析及病死率[5]。多種風(fēng)險(xiǎn)因素可以促進(jìn)CIN的發(fā)生,如基礎(chǔ)的腎功能不全、低血壓、心力衰竭、糖尿病、老年、貧血、造影劑的數(shù)量和類(lèi)型等[6-10]。已經(jīng)明確CIN與患者的近期及遠(yuǎn)期預(yù)后相關(guān),目前臨床上習(xí)慣應(yīng)用水化療法預(yù)防CIN的發(fā)生[11], 但能確切有效防治CIN的措施非常有限, 所以識(shí)別 CIN的危險(xiǎn)因素成為防治CIN的基石[12-13]。本研究對(duì)行CAG或非急診PCI的不穩(wěn)定型心絞痛患者,在水化治療的基礎(chǔ)上,探討CIN的發(fā)生率及其危險(xiǎn)因素,以期提高臨床醫(yī)生對(duì)CIN的重視,旨在提高患者生存率及遠(yuǎn)期預(yù)后?,F(xiàn)報(bào)告如下。
1.1一般資料選擇2012年10月—2015年10月于我院心血管內(nèi)科住院、擬行CAG或非急診PCI的不穩(wěn)定型心絞痛患者500例,男性337例,女性163例,年齡18~80歲,平均(64.6±8.1)歲;其中CAG 175例,PCI 325例。
本研究經(jīng)醫(yī)院倫理委員會(huì)通過(guò),所有患者均簽署知情同意書(shū)。
1.2相關(guān)定義和計(jì)算公式
1.2.1CINCIN定義為應(yīng)用對(duì)比劑后48 h內(nèi)出現(xiàn)血清肌酐(serum creatinine,SCr)上升,相對(duì)值超過(guò)基礎(chǔ)值的25%或絕對(duì)值超過(guò)基礎(chǔ)值44 μmol/L。
1.2.2腎小球?yàn)V過(guò)率(estimated glomerular filtration rate,eGFR)計(jì)算公式eGFR計(jì)算公式:eGFR=186.3(SCr)-1.154(年齡)-0.203(女性:×0.742)。
1.2.3Mehran評(píng)分采用CIN危險(xiǎn)因素風(fēng)險(xiǎn)評(píng)分系統(tǒng),見(jiàn)表1。

1.3方法入選患者于CAG或非急診PCI前后進(jìn)行水化治療(術(shù)前12 h至術(shù)后12 h靜脈輸注0.9%氯化鈉1.0 m L·kg-1·h-1),造影劑選用碘普羅胺注射液(優(yōu)維顯)。于CAG或非急診PCI術(shù)前,術(shù)后24 h、48 h、72 h及7 d測(cè)定患者的胱抑素C(Cystatin C,Cys C)、SCr及eGFR,計(jì)算入選患者CIN的發(fā)生率。并依據(jù)是否發(fā)生CIN,將患者分為CIN組及非CIN組,比較2組的臨床特點(diǎn),包括人口學(xué)特征(性別、年齡、身高、體質(zhì)量等)、病史(高血壓、糖尿病、高脂血癥等)、實(shí)驗(yàn)室數(shù)據(jù)(SCr、Cys C、eGFR、糖化血紅蛋白、血脂等)、藥物使用(不穩(wěn)定型心絞痛的常規(guī)治療如阿司匹林、氯吡格雷、硝酸酯類(lèi)、低分子肝素、他汀類(lèi)藥物等)、冠狀動(dòng)脈病變嚴(yán)重程度、手術(shù)操作方式、造影劑用量等。評(píng)估CIN組CAG或非急診PCI前后SCr、eGFR及Cys C等腎功能指標(biāo)的變化情況。對(duì)2組進(jìn)行CIN危險(xiǎn)評(píng)分(Mehran評(píng)分系統(tǒng)),分析CIN發(fā)生的危險(xiǎn)因素。
2.1CIN的發(fā)生率500例入選患者中發(fā)生CIN 72例,發(fā)生率為14.4%,其中行CAG者17例,行PCI者55例。依據(jù)是否發(fā)生CIN將患者分為CIN組72例及非CIN組428例。CIN組年齡、入院時(shí)Cys C水平、入院時(shí)SCr水平、造影劑用量、糖尿病比例、高血壓比例、病變嚴(yán)重程度比例、Mehran評(píng)分均高于非CIN組,eGFR低于非CIN組,差異有統(tǒng)計(jì)學(xué)意義(P<0.05);2組性別、體質(zhì)量指數(shù)、糖化血紅蛋白、三酰甘油、總膽固醇、低密度脂蛋白膽固醇、高密度脂蛋白膽固醇、左心室射血分?jǐn)?shù)以及氯吡格雷、硝酸酯、低分子肝素、他汀類(lèi)使用率差異均無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05)。見(jiàn)表2。

2.2CIN組腎功能的變化情況CIN組Cys C水平于CAG或非急診PCI后24 h升高達(dá)峰值,48 h開(kāi)始回落,至72 h恢復(fù)至術(shù)前水平;SCr水平于CAG或非急診PCI后24 h開(kāi)始升高,48 h達(dá)峰值,72 h逐漸回落,7 d基本恢復(fù)至術(shù)前水平;eGFR水平于CAG或非急診PCI后24 h開(kāi)始降低,48 h達(dá)最低水平,72 h逐漸回升,7 d基本恢復(fù)至術(shù)前水平。CIN組手術(shù)前后腎功能指標(biāo)差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。見(jiàn)表3。

2.3Logistic回歸分析以CIN為因變量,以年齡、入院Cys C水平、入院SCr水平、入院eGFR水平、糖尿病病史、高血壓病史、造影劑用量、冠狀動(dòng)脈病變嚴(yán)重程度、手術(shù)操作方式及Mehran評(píng)分為自變量,進(jìn)行多因素Logistic回歸分析,結(jié)果顯示入院Cys C水平、入院eGFR水平、糖尿病病史、造影劑用量及Mehran評(píng)分是CIN的危險(xiǎn)因素。見(jiàn)表4,5。

隨著CAG和PCI在心血管疾病診斷和治療領(lǐng)域的發(fā)展,CIN的發(fā)生率逐漸升高,已成為心臟介入手術(shù)中繼支架術(shù)后“血栓形成”及支架術(shù)后“再狹窄”的又一難題。CIN的發(fā)生可以導(dǎo)致住院時(shí)間延長(zhǎng),增加患者近、遠(yuǎn)期透析風(fēng)險(xiǎn)及病死率。本研究在水化治療的基礎(chǔ)上,觀察500例行CAG或非急診PCI不穩(wěn)定型心絞痛患者CIN的發(fā)生率,結(jié)果顯示CIN發(fā)生率為14.4%。提示CIN在行CAG或非急診PCI的不穩(wěn)定型心絞痛患者中很常見(jiàn),即使預(yù)防性應(yīng)用水化治療,發(fā)病率仍然很高。
CIN通常用SCr值來(lái)評(píng)價(jià),目前最通用的定義是使用對(duì)比劑后48 h內(nèi)出現(xiàn)SCr上升,超過(guò)其基礎(chǔ)值的25%或超過(guò)44 μmol/L。SCr評(píng)價(jià)CIN特異度高,但因其受到性別、飲食、肌肉含量、活動(dòng)等影響,使其敏感度受到了一定的限制,在大多數(shù)腎臟損傷初期,其診斷缺乏一定的準(zhǔn)確度。Cys C是除血清SCr之外的一個(gè)新的反映eGFR變化的內(nèi)源性標(biāo)志物,且不受性別、年齡、體質(zhì)量、飲食等影響,故其血清水平主要由eGFR決定,可以準(zhǔn)確反映早期的腎功能損傷[14-15]。對(duì)于慢性腎臟疾病患者,PCI術(shù)后24 h Cys C水平增高>10%,對(duì)評(píng)估CIN發(fā)生的敏感度和特異度分別達(dá)到100%和86%[16]。因此,將Cys C與SCr結(jié)合起來(lái),可以提高CIN診斷的靈敏度與準(zhǔn)確度。CIN的危險(xiǎn)因素包括內(nèi)源性危險(xiǎn)因素和外源性危險(xiǎn)因素[17]。常見(jiàn)的內(nèi)源性危險(xiǎn)因素包括原有腎功能不全、低血壓、充血性心力衰竭、高齡、糖尿病等[18]。外源性危險(xiǎn)因素包括造影劑的類(lèi)型與劑量、注入造影劑的途徑(動(dòng)脈或靜脈),以及2次造影劑使用的間隔時(shí)間等[19]。本研究結(jié)果顯示,造影劑的用量、糖尿病病史、入院Cys C增高、eGFR降低及Mehran評(píng)分是CIN的獨(dú)立預(yù)測(cè)因子,其中
Mehran評(píng)分的相關(guān)性最好。
為了評(píng)估CIN危險(xiǎn)性及對(duì)其進(jìn)行危險(xiǎn)分層,Mehran等在2004年建立了CIN危險(xiǎn)因素的簡(jiǎn)化評(píng)分系統(tǒng),危險(xiǎn)積分≤5分時(shí),CIN發(fā)生風(fēng)險(xiǎn)為7.5%,血液透析風(fēng)險(xiǎn)為0.04%;危險(xiǎn)積分為6~10分時(shí),CIN發(fā)生風(fēng)險(xiǎn)為14%,血液透析風(fēng)險(xiǎn)為0.12%;危險(xiǎn)積分為11~16分時(shí),CIN發(fā)生風(fēng)險(xiǎn)為26.1%,血液透析風(fēng)險(xiǎn)為1.09%;當(dāng)危險(xiǎn)積分>16分時(shí)CIN的發(fā)生風(fēng)險(xiǎn)為57.3%,血液透析風(fēng)險(xiǎn)為12.6%[20]。Mehran評(píng)分囊括了大部分CIN可能的危險(xiǎn)因素,評(píng)估CIN較為準(zhǔn)確全面。因此,對(duì)于擬行CAG或PCI的患者,尤其是高?;颊撸紤?yīng)該行Mehran評(píng)分進(jìn)行危險(xiǎn)分層,對(duì)評(píng)分高的患者給予積極的預(yù)防措施,以減少CIN的發(fā)生。在條件允許的情況下,盡量減少造影劑的用量,以減少CIN的發(fā)生。

Analysis of risk factors of contrast-induced nephropathy in patients undergoing coronary angiography or non-emergent percutaneous coronary intervention

ObjectiveTo observe the renal functional variance before and after coronary angiography(CAG) or non-emergent percutaneous coronary intervention(PCI) in patients with unstable angina, to determine the incidence rate of contrast-induced nephropathy(CIN) and to discuss the risk factors. MethodsFive hundred patients with unstable angina enrolled into the study. Cystatin C(Cys C), serum creatinine(SCr) and estimated glomerular filtration rate(eGFR) were collected before and after CAG or non-emergent PCI at different time points on the basis of the treatment of hydration to estimate the incidence of CIN. The patients were divided into CIN group and non-CIN group, and the changes of renal function before and after operation in group CIN were evaluated. The clinical characteristics of the two groups were compared, and the risk factors of the occurrence of CIN were analyzed by CIN risk score(Mehran scoring system). ResultsCIN occurred in 72 of 500 patients, and the incidence was 14.4%. The factors of age, baseline Cys C level, baseline SCr level, the dosage of contrast medium, hypertension history, diabetic history, severity of coronary lesions, Mehran scores of CIN group were higher than that of non-CIN group, and baseline eGFR level were lower than that of non-CIN group. The difference was statistically significant(P<0.05). The level of Cys C and SCr in patients with CIN after operation showed a trend of first increasing and then decreasing and the eGFR level was decreased and then increased. The difference was statistically significant(P<0.05). The results of multivariate Logistic regression analysis showed that: baseline Cys C level, baseline eGFR level, diabetic history, the dosage of contrast medium, and Mehran scores were the risk factors of CIN. ConclusionThe incidence of CIN is still high, even with the preventive use of the treatment of hydration. The detection rate will be increased if Cys C and SCr are combined. Diabetes history, baseline renal dysfunction, the dosage of contrast medium and Mehran score are independent risk factors for CIN, and the most relevant risk factor is Mehran score, which can be applied before operation.
