槐耳顆粒在鼻咽癌放化療患者中的應(yīng)用效果

[摘要]目的 探討槐耳顆粒在鼻咽癌放化療患者中的應(yīng)用效果。方法 選取2014年1月～2016年1月我院初治的同期放化療鼻咽癌患者60例作為研究對象，采用簡單隨機化法將其分為對照組和觀察組，每組各30例。對照組患者給予常規(guī)同期放化療，觀察組患者在同期放化療基礎(chǔ)上聯(lián)合槐耳顆粒，治療完成后1個月，比較分析兩組患者治療前后T細(xì)胞亞群表達變化、臨床療效及生存質(zhì)量。結(jié)果 兩組患者的臨床治療總有效率比較，差異無統(tǒng)計學(xué)意義（P>0.05）;治療后，兩組患者的CD3+、CD4+、CD4+/CD8+細(xì)胞表達水平均高于治療前，且觀察組患者的CD3+、CD4+、CD4+/CD8+細(xì)胞表達水平高于對照組，差異有統(tǒng)計學(xué)意義（P<0.05）;治療后，兩組患者的CD8+細(xì)胞表達水平均低于治療前，觀察組患者的CD8+細(xì)胞表達水平低于對照組，差異有統(tǒng)計學(xué)差異（P<0.05）;同期放化療后，觀察組患者的軀體功能評分高于對照組，且疲勞、惡心/嘔吐及疼痛評分低于對照組，差異均有統(tǒng)計學(xué)意義（P<0.05）。結(jié)論 槐耳顆粒能有效提高鼻咽癌同期放化療后的免疫功能，還能改善患者的生存質(zhì)量，臨床應(yīng)用價值較高。

[Abstract] Objective To explore the effect of Huai′er Granules in radiochemotherapy for patients with nasopharyngeal carcinoma. Methods A total of 60 patients with nasopharyngeal carcinoma who underwent concurrent radiotherapy and chemotherapy from January 2014 to January 2016 were selected as the research objects. The selected patients were divided into the control group and the observation group by simple randomization， 40 cases in each group. The control group was treated with concurrent chemoradiotherapy， and the observation group was treated with concurrent chemoradiotherapy combined with Huai′er Granules. One month after the treatment was completed， the changes in T cell expression， clinical efficacy， and quality of life of patients of the two groups before and after treatment were analyzed. Results The total effective rate of clinical treatment of patients between the two groups was not statistically significant （P>0.05）. After treatment， the expression levels of CD3+， CD4+， CD4+/CD8+ cells in two groups were higher than before treatment， and the expression levels of CD3+， CD4+， CD4+/CD8+ cells in the observation group were higher than those in control group， the differences were statistically significant （P<0.05）. After treatment， the expression level of CD8+ cells in two groups were lower than those before treatment， and the expression level of CD8+ cells in the observation group was lower than that in control group （P<0.05）. After radiochemotherapy， the physical function score of the observation group was higher than that in the control group， and the meanwhile， the fatigue， nausea/vomiting and pain scores in observation group were lower than those in control group， the differences were statistically significant （P<0.05）. Conclusion Huai′er Granule can effectively improve the immune function of nasopharyngeal carcinoma after radiotherapy and chemotherapy at the same time， and can also improve the quality of life of patients. It is worthy of further clinical research.

槐耳是我國民間常用的補中益氣藥物，對提高腫瘤患者低下的免疫力具有顯著作用，可誘導(dǎo)人體產(chǎn)生α、γ干擾素，誘導(dǎo)T淋巴細(xì)胞分裂、繁殖、成熟、分化，調(diào)整抑制性與輔助性T淋巴細(xì)胞的比例，產(chǎn)生某些細(xì)胞激活因子，如白細(xì)胞介素-2，使NK細(xì)胞活性增強，是具有明顯協(xié)同作用的免疫調(diào)節(jié)劑[6]?；倍w粒還可作為抗腫瘤常用的輔助藥物，臨床可配合手術(shù)和放、化療等治療手段，在腫瘤的治療過程中有明顯的抑制作用，增強機體免疫功能，提高惡性腫瘤放化療后患者的生存質(zhì)量[7]。

腫瘤的形成及發(fā)展過程中，CD4+/CD8+T淋巴細(xì)胞比例降低導(dǎo)致機體免疫功能低下，并協(xié)同促進腫瘤的進一步惡化進展?；倍哂酗@著促進T淋巴細(xì)胞增殖的作用，能提高惡性腫瘤患者T細(xì)胞亞群百分比及CD4+/CD8+T淋巴細(xì)胞比值[9-10]?；倍?lián)合其他藥物用于治療腫瘤取得了較好的臨床效果。以西羅莫司為基礎(chǔ)，聯(lián)合槐耳顆粒、胸腺肽α1的三聯(lián)抗腫瘤療法可抑制Treg細(xì)胞的表達和增殖，通過抑制細(xì)胞間的相互作用，促進肝移植復(fù)發(fā)模型大鼠的T淋巴細(xì)胞數(shù)量升高，提高CD4+/CD8+比值[11-12]?；颊咝g(shù)后服用槐耳顆粒，同樣能提高T淋巴細(xì)胞亞群百分比，增大CD4+/CD8+細(xì)胞比值[13-14]。因此，槐耳顆粒能通過升高T淋巴細(xì)胞以達到提高機體抗腫瘤的免疫功能，從而改善患者預(yù)后[15]。在本研究中，兩組鼻咽癌患者治療后的總有效率比較，差異無統(tǒng)計學(xué)意義（P>0.05）;同期放化療前兩組患者的CD3+、CD4+、CD8+、CD4+/CD8+細(xì)胞表達水平比較，差異無統(tǒng)計學(xué)意義（P>0.05）;行同期放化療后，兩組患者的CD3+、CD4+、CD4+/CD8+細(xì)胞表達水平均高于治療前，CD8+細(xì)胞表達水平低于治療前，差異有統(tǒng)計學(xué)意義（P<0.05）。提示患者機體免疫功能下降與腫瘤的發(fā)生和發(fā)展關(guān)系密切，針對鼻咽癌同期放化療聯(lián)合槐耳顆粒治療后可減輕腫瘤負(fù)荷，促進免疫功能抑制得到緩解;同期放化療聯(lián)合槐耳顆粒治療后觀察組患者的CD3+、CD4+、CD4+/CD8+細(xì)胞表達水平高于對照組，CD8+細(xì)胞表達水平低于對照組，差異有統(tǒng)計學(xué)意義（P<0.05），提示其能進一步提高鼻咽癌患者治療的免疫功能。本研究結(jié)果還顯示，同期放化療聯(lián)合槐耳顆粒治療的鼻咽癌患者功能子量表中的軀體功能得分高于常規(guī)放化療的對照組患者（P<0.05），而癥狀子量表中的疲勞、惡心/嘔吐和疼痛得分低于對照組（P<0.05），提示槐耳顆粒對患者機體狀態(tài)的保護作用及其提高生存質(zhì)量有重要意義。

綜上所述，槐耳顆粒雖不能顯著提高鼻咽癌患者放化療的臨床療效，但能有效提高鼻咽癌同期放化療后的免疫功能，改善患者的生存質(zhì)量，臨床應(yīng)用價值較高。
