百令膠囊聯(lián)合脈血康治療2型糖尿病早期腎病的臨床研究

【摘要】 目的 探討百令膠囊聯(lián)合脈血康治療2型糖尿病早期腎病對(duì)患者腎臟生化指標(biāo)的影響。方法 100例2型糖尿病早期腎病患者， 采用隨機(jī)數(shù)字表法分為試驗(yàn)組和對(duì)照組， 每組50例。對(duì)照組采用常規(guī)治療， 試驗(yàn)組在常規(guī)治療基礎(chǔ)上給予百令膠囊聯(lián)合脈血康治療， 觀察兩組在治療前、治療后6個(gè)月、治療后1年的腎臟生化指標(biāo)改變情況。結(jié)果 治療前試驗(yàn)組24 h尿白蛋白排泄率（UAER）、尿素（UR）、肌酐（Cr）、胱抑素C（CysC）指標(biāo)分別為（298.7±16.6）mg/d、（7.6±0.9）mmol/L、（164.8±15.8）μmol/L、（2.9±0.9）mg/L， 與對(duì)照組的（289.6±15.8）mg/d、（7.6±0.8）mmol/L、（170.5±14.9）μmol/L、（2.8±0.9）mg/L比較， 差異無統(tǒng)計(jì)學(xué)意義（P>0.05）。治療6個(gè)月及治療1年后， 兩組上述指標(biāo)均優(yōu)于治療前， 差異有統(tǒng)計(jì)學(xué)意義（P<0.05）；治療1年后， 試驗(yàn)組UAER、CysC指標(biāo)優(yōu)于對(duì)照組， 差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論 百令膠囊聯(lián)合脈血康膠囊治療2型糖尿病早期腎病可以顯著改善患者的早期癥狀， 較單純傳統(tǒng)療法效果更佳。

【Abstract】 Objective To investigate influence by Bailing capsules combined with Maixuekang capsules on renal biochemical indexes in the treatment of type 2 early diabetic nephropathy. Methods A total of 100 patients with type 2 early diabetic nephropathy were divided by random number table into experimental group and control group， with 50 cases in each group. The control group received conventional therapy， and the experimental group received additional Bailing capsules combined with Maixuekang capsules for treatment on the basis of conventional therapy. Observation was made on changes of renal biochemical indexes in both groups before treatment and in 6 months and 1 year after treatment. Results Before treatment， the experimental group had 24 h urinary albumin excretion rate （UAER）， urea （UR）， creatinine （Cr）， and cystatin C （CysC） respectively as （298.7±16.6） mg/d， （7.6±0.9） mmol/L， （164.8±15.8） μmol/L and （2.9±0.9） mg/L. Comparing with （289.6± 15.8） mg/d， （7.6±0.8） mmol/L， （170.5±14.9） μmol/L and （2.8±0.9） mg/L in the control group， their difference had no statistical significance （P>0.05）. In 6 months and 1 year after treatment， both groups had better indexes than those before treatment， and the difference had statistical significance （P<0.05）. In 1 year after treatment， the experimental group had better UAER and CysC than the control group， and the difference had statistical significance （P<0.05）. Conclusion Combination of Bailing capsules and Maixuekang capsules in treating type 2 early diabetic nephropathy can remarkably improve early symptoms in patients， and it is better than conventional therapy.

糖尿病是一組以血糖代謝紊亂為主要臨床表現(xiàn)的代謝性疾病， 患者早期無任何不適癥狀， 往往通過體檢、不明原因消瘦、多尿、口渴等癥狀后進(jìn)行檢查而被發(fā)現(xiàn)， 而此時(shí)患者往往經(jīng)過了較長(zhǎng)病程且伴隨有不同程度的并發(fā)癥， 其中糖尿病腎病就是最為常見的并發(fā)癥。糖尿病腎病是主要的糖尿病微血管病變之一， 常見于病史>10年的患者， 由其引發(fā)的終末期腎功能衰竭是糖尿病患者致死、致殘的主要原因[1]。研究顯示， 2型糖尿病并發(fā)糖尿病腎病的患病率已達(dá)到34.7%。糖尿病腎病的基本病理特征主要表現(xiàn)為腎小球基底膜增厚、腎小球硬化以及細(xì)胞外基質(zhì)增多[2]， 目前其發(fā)病機(jī)制尚不清楚， 公認(rèn)的觀點(diǎn)認(rèn)為因內(nèi)分泌紊亂導(dǎo)致糖脂代謝失調(diào)或與高血壓、血流動(dòng)力學(xué)障礙等有關(guān)。對(duì)早期糖尿病腎病患者采取積極藥物干預(yù)可有效緩解疾病進(jìn)展。既往研究表明， 百令膠囊以及脈血康膠囊對(duì)糖尿病腎病患者有一定的治療效果， 臨床實(shí)際中單藥療效仍不理想， 采用聯(lián)合藥物治療糖尿病腎病療效可能有疊加作用， 然而兩藥聯(lián)合使用對(duì)糖尿病腎病的治療作用卻少見報(bào)道。本課題通過在常規(guī)治療基礎(chǔ)上增加百令膠囊聯(lián)合脈血康對(duì)2型糖尿病早期腎病患者進(jìn)行綜合干預(yù)， 以期為該類患者治療提供參考。
