【摘 要】冠心病是嚴重威脅人類健康的心血管病之一,目前,其已被認為是一種慢性炎癥疾病.近幾年研究發(fā)現,血常規(guī)中一些指標,如NLR、PLR、MLR等作為新型炎癥指標,能反映及預測冠心病的發(fā)生發(fā)展。

Abstract: objective: To coronary heart disease is one of the cardiovascular diseases that threaten human health. Currently, it has been considered as a chronic inflammatory disease [2]. Studies in recent years have found that some indexes in blood routine, such as NLR, PLR and MLR, can reflect and predict the occurrence and development of coronary heart disease as new inflammatory indicators.

在我國,CHD的發(fā)病率一直居高不下,病死率逐年上升.近幾年,炎癥已成為CHD中的研究熱點;目前已證實,血常規(guī)中的白細胞及其分類(中心粒細胞、淋巴細胞、單核細胞)作為炎癥指標在CHD的病程中起到重要作用[1]然而,由于白細胞及其分類易受到各種生理、病理因素的影響,其對冠心病的預測價值有限.而血常規(guī)中一些指標,如中心粒細胞與淋巴細胞比值(NLR)、血小板與淋巴細胞比值(PLR)、單核細胞與淋巴細胞比值(MLR)與已被證實是CHD的預測因子的炎性指標有著強烈的關聯性,提示其對CHD有一定的預測作用,且相對于單一指標來說,這些指標更加穩(wěn)定.本文將這些指標在冠心病中的預測價值做一綜述.
1 NLR在CHD中的預測價值
NLR是中心粒細胞與單核細胞的比值,反映了這兩種炎癥細胞的平衡.隨著研究的深入,近幾年發(fā)現NLR能預測CHD的發(fā)生及反映其嚴重程度.一項納入了7017CHD病例的Mete分析顯示[2],NLR與冠脈病變程度及CHD嚴重程度呈正相關,且對CHD有一定的預測價值.在Ibrahim Sari[11]研究中顯示,NLR是CHD的獨立預測因子,當NLR>2.3時,其預測CHD發(fā)生的敏感性為66%,特異性為70%.胡楊[3]等人的研究表明,不同冠心病類型其 NLR水平不同.國內一項前瞻性研究報道[4],NLR是預測心肌梗死合并冠脈多支病變遠期預后的獨立預測因子.有研究報道[5],NLR還可通過預測CHD患者的冠脈血流儲備,繼而預測其預后。
2 PLR在CHD中的預測價值
目前,關于PLR與CHD的研究,各研究報道不一.Murat Yüksel等人[6]的研究顯示,PLR與CHD獨立相關,PLR診斷CHD的ROC曲線下面積為64.4%,具有良好的預測價值. 然而,在Carlos V. Serrano Jr[7]的研究報道中,PLR并不是一個獨立預測因子.在一篇報道中,按年齡進行分層后分析,發(fā)現<45歲人群的NLR高于>45歲人群,且PLR對<45歲人群的預測效能高于>45歲人群.另外,在Cuneyt Toprak[8]研究中發(fā)現,在CHD中,PLR是一個血栓形成的指示物,其可以預測病變部位血栓的形成要原因;在DONG ZHOU[9]的研究中證實了PLR是MACE發(fā)生的獨立預測因子.除此,有研究報道,PLR還能預測心肌梗死患者的冠脈開放程度,而冠脈開放程度決定了心?;颊呤中g的成功率、住院死亡率及心肌活力.
3 MLR在CHD中的預測價值
最近幾項研究提示,MLR在預測CHD的嚴重程度及預后方面是一個良好的預測指標.在一項研究中[10],N. KOSE等人發(fā)現MLR是冠脈病變的獨立預測因子.Zeyuan Fan[11]等人的研究顯示,在NSTEMI 患者中,MLR是嚴重不良心血管事件的獨立預測因子(ROC AUC=68.3%),可用于鑒別預后不良的高?;颊?有研究報道,MLR是CHD患者行PCI術后冠脈再狹窄的獨立預測因子.另外他們在另一項研究中[12]首次報道了穩(wěn)定性心絞痛(Stable Angina,SA)患者MLR水平和斑塊易損性之間的關系,他指出,MLR水平升高是易損斑塊形成的獨立危險因素,當MLR取截斷值0.25時,其具有良好的預價值;另外,他還發(fā)現MLR水平與易損板塊中起重要作用的成分壞死核心((necrotic core,NC)數量呈正相關.
4 總結與展望
血常規(guī)中的NLR、PLR、LMR等作為新型炎癥指標,在CHD及預后的預測價值方面具有重要作用,但目前一些研究結果仍存在著差異.在未來的工作中,我們需要進一步擴大樣本量、進行多中心的前瞻性研究,讓血細胞新型炎癥指標在CHD中的預測價值得以完善.
