[摘要]目的:探討濃縮生長(zhǎng)因子纖維蛋白(Concentrated growth factor fibrin,CGF)治療牙齦退縮的效果及對(duì)牙齦成纖維細(xì)胞(Human gingival fibroblast,HGF)增殖的影響。方法:選擇2018年6月-2019年6月筆者醫(yī)院收治的30例牙齦退縮患者為研究對(duì)象,采集患者靜脈血制備CGF膜,將CGF膜固定于術(shù)區(qū)進(jìn)行治療,觀察術(shù)前及術(shù)后6個(gè)月的牙周袋深度(Pocket depth,PD)、退縮高度(Heigh of recession,HR)、角化齦寬度(Keratinized mucosa width,KMW)、紅色美學(xué)評(píng)分(Pink aesthetic score,PES)和白色美學(xué)評(píng)分(White aesthetics score,WES)。取正常牙齦組織,采用貼壁法傳代培養(yǎng)成HGF,在電鏡下觀察CGF膜上HGF的黏附情況,采用CCK-8法檢測(cè)HGF的增殖情況。結(jié)果:與術(shù)前比較,術(shù)后6個(gè)月患者的PD[(2.89±0.57)mm vs (2.58±0.36)mm]、KMW[(3.99±0.68)mm vs (3.55±0.64)mm]、PES[(7.42±1.63)分 vs (2.80±0.59)分]、WES[(7.90±1.74)分 vs (2.77±0.55)分]均明顯增高(P<0.05),HR[(1.95±0.56)mm vs (2.97±0.48)mm]明顯降低(P<0.05)。通過(guò)電鏡能夠清晰看到HGF呈層層黏附于CGF膜上;HGF的增殖檢測(cè)結(jié)果顯示,培養(yǎng)1d、4d、7d時(shí),CGF組的OD值[(0.54±0.08) vs (0.34±0.07)]、[(0.85±0.12) vs (0.55±0.14)]、[(1.13±0.30) vs (0.66±0.18)]均明顯高于對(duì)照組(P<0.05)。結(jié)論:CGF治療牙齦退縮的臨床效果及美學(xué)效果均顯著,能夠明顯促進(jìn)HGF的黏附、生長(zhǎng)和增殖。

Curative Effect of Concentrated Growth Factor Fibrin in the Treatment of Gingival Recession and its Impact on Proliferation of Human Gingival Fibroblasts

Abstract:Objective? To discuss the curative effect of concentrated growth factor fibrin(CGF) in the treatment of gingival recession and its impact on proliferation of human gingival fibroblasts(HGF). Methods? 30 patients with gingival recession who was taken as research objects in our hospital from June 2018 to June 2019. Venous blood was collected to prepare CGF membrane, which was fixed in the operative area for treatment. The pocket depth(PD), heigh of recession(HR), keratinized mucosa width(KMW), pink aesthetic score(PES),white aesthetics score(WES) was observed before operation and 6 months after operation. The adhesion of HGF on CGF membrane was observed under electron microscopy. The proliferation of HGF was detected by CCK-8 method. Results Compared to before operation, the PD[(2.89±0.57)mm vs (2.58±0.36)mm],KMW[(3.99±0.68)mm vs (3.55±0.64)mm], PES[(7.42±1.63)scores vs (2.80±0.59)scores], WES[(7.90±1.74)scores vs (2.77±0.55)scores] of patients were significantly increased and the HR[(1.95±0.56)mm vs (2.97±0.48)mm] of patients were significantly decreased 6 months after operation(P<0.05). By means of electron microscopy, it was clear that HGF adhered to CGF membranelayer by layer. The results of proliferation detection of HGF showed, cultured for 1d, 4d and 7d, the OD value[(0.54±0.08) vs (0.34±0.07)], [(0.85±0.12) vs (0.55±0.14)], [(1.13±0.30) vs (0.66±0.18)] of CGFgroup was significantly higher than that of control group(P<0.05). Conclusion The clinical and aesthetic effects were significant of CGF in treatment of gingival recession. CGF could significantly promote the adhesion, growth, proliferation of HGF.
膜齦術(shù)式繁多,其最終目的都是為了獲取完全的根面覆蓋和良好的美觀效果,國(guó)內(nèi)外學(xué)者將不同術(shù)式聯(lián)合不同的生物材料,輔助于促進(jìn)牙齦軟組織的愈合[10]。人牙齦成纖維細(xì)胞是人牙齦結(jié)蹄組織中的主要細(xì)胞,主要存在于牙齦固有層中,其主要功能是合成膠原纖維、彈性纖維及基質(zhì),具有較活躍的自身更新能力,在牙周軟組織的自身修復(fù)及創(chuàng)傷愈合中起著至關(guān)重要的作用[15]。有研究[16]分析兩種富血小板纖維蛋白產(chǎn)物對(duì)于牙齦成纖維細(xì)胞的影響,結(jié)果表明富血小板纖維蛋白膜對(duì)于HGF增殖有促進(jìn)作用。本研究還通過(guò)體外實(shí)驗(yàn)探討了CGF對(duì)HGF增殖的影響。結(jié)果顯示,通過(guò)電鏡能夠清晰看到HGF呈層層黏附于CGF膜上,HGF的增殖檢測(cè)中,培養(yǎng)1d、4d、7d時(shí),CGF組的OD值均明顯高于對(duì)照組。說(shuō)明CGF體外促進(jìn)HGF的效果較好,并證實(shí)了CGF在促進(jìn)HGF的增殖中起重要作用。
綜上所述,CGF具有來(lái)源豐富、高效安全,無(wú)排斥反應(yīng)等優(yōu)點(diǎn)。CGF治療牙齦退縮的臨床效果及美學(xué)效果均顯著,能夠明顯促進(jìn)HGF的黏附、生長(zhǎng)及增殖,這一結(jié)果對(duì)臨床應(yīng)用CGF輔助于牙齦促進(jìn)軟組織再生提供了重要的理論依據(jù)。
