肺癌化療患者PICC置管中護理的應(yīng)用效果評價

【摘要】 目的 分析肺癌化療患者外周靜脈置入中心靜脈導(dǎo)管（PICC）置管中護理的應(yīng)用效果。方法 60例肺癌化療患者， 依據(jù)入院的先后順序分為參照組和研究組， 每組30例。參照組患者接受常規(guī)護理， 研究組患者接受PICC置管護理， 比較兩組患者的護理效果。結(jié)果 研究組患者護理滿意度為96.7%， 高于參照組的66.7%， 差異有統(tǒng)計學(xué)意義（χ2=9.017， P<0.05）。研究組患者生理領(lǐng)域評分為（9.46±1.24）分， 心理領(lǐng)域評分為（13.53±2.26）分， 社會關(guān)系評分為（13.55±2.26）分， 環(huán)境領(lǐng)域評分為（12.87±2.32）分， 高于參照組的（6.59±1.16）、（11.55±1.49）、（10.55±1.52）、（9.19±1.86）分， 差異有統(tǒng)計學(xué)意義（t=9.2577、4.0063、6.0331、6.7785， P<0.05）。研究組患者并發(fā)癥發(fā)生率為6.67%， 低于參照組的26.67%， 差異有統(tǒng)計學(xué)意義（χ2=4.3200， P<0.05）。結(jié)論 肺癌化療患者應(yīng)用PICC置管護理， 可以明顯降低并發(fā)癥發(fā)生率， 使患者的生活質(zhì)量有所提升， 具有臨床應(yīng)用價值。

【Abstract】 Objective To analyze application effect of peripherally inserted central catheter （PICC） insertion nursing on lung cancer chemotherapy patients. Methods A total of 60 lung cancer chemotherapy patients were divided by admission order into control group and research group， with 30 cases in each group. The control group received conventional nursing， and the research group received PICC insertion nursing. Nursing effect was compared in two groups. Results The research group had higher nursing satisfaction degree as 96.7% than 66.7% in the control group， and the difference had statistical significance （χ2=9.017， P<0.05）. The research group had physical domain score as （9.46±1.24）points， psychological domain score as （13.53±2.26）points， social relationship score as （13.55±2.26）points， and environmental domain score as （12.87±2.32）points， which were all higher than （6.59±1.16）， （11.55±1.49）， （10.55±1.52） and （9.19±1.86）points in the control group， and their difference had statistical significance （t=9.2577， 4.0063， 6.0331， 6.7785， P<0.05）. The research group had lower incidence of complications as 6.67% than 26.67% in the control group， and the difference had statistical significance （χ2=4.3200， P<0.05）. Conclusion Implement of PICC insertion nursing in lung cancer chemotherapy patient can clearly reduce the incidence of complications and improve quality of life. So this method contains clinical value of application.

PICC近年來廣泛應(yīng)用于臨床靜脈輸液中。此方法操作簡單， 并且留置時間較長[1]。但是在臨床護理過程中， 因相關(guān)因素的影響， 患者極易出現(xiàn)并發(fā)癥， 如靜脈炎、穿刺點感染以及導(dǎo)管堵塞等， 為此需要對其進行針對性護理以降低并發(fā)癥發(fā)生率[2]。為尋求有效的護理對策， 選擇本科收治的60例肺癌化療患者進行實驗研究， 結(jié)果報告如下。

1 資料與方法

1. 1 一般資料 選擇本科2014年7月～2016年6月收治的60例肺癌化療患者作為研究對象， 依據(jù)入院的先后順序分為參照組和研究組， 每組30例。研究組患者中男22例， 女8例；年齡最大70歲， 最小42歲， 平均年齡（54.8±5.6）歲。參照組患者中男21例， 女9例， 年齡最大75歲， 最小42歲， 平均年齡（56.2±6.8）歲。兩組患者一般資料比較， 差異無統(tǒng)計學(xué)意義（P>0.05）， 具有可比性。
