摘要:目的 探討丙氨酸轉(zhuǎn)氨酶(ALT)/堿性磷酸酶(ALP)比值、血小板計(jì)數(shù)(PLT)與淋巴細(xì)胞(LYM)比值(PLR)對老年膿毒癥休克并發(fā)肝損傷患者預(yù)后的預(yù)測價值。方法 回顧性分析116例膿毒癥休克并發(fā)肝損傷患者的臨床資料,根據(jù)肝損傷程度分為輕度組39例、中度組47例和重度組30例。并根據(jù)患者入院28 d的存活情況將患者分為死亡組69例和存活組47例。收集急性生理與慢性健康Ⅱ(APACHEⅡ)評分、序貫器官衰竭(SOFA)評分;檢測患者外周血中PLT、LYM,計(jì)算PLR;測定ALT、ALP,計(jì)算ALT/ALP比值;檢測白細(xì)胞計(jì)數(shù)、血乳酸、C反應(yīng)蛋白等實(shí)驗(yàn)室指標(biāo)。采用多因素Logistic回歸分析膿毒癥休克并發(fā)肝損傷患者預(yù)后的影響因素,受試者工作特征(ROC)曲線分析ALT/ALP比值、PLR對膿毒癥休克并發(fā)肝損傷患者預(yù)后的預(yù)測價值。結(jié)果 輕度組、中度組和重度組血清ALT、ALP、ALT/ALP比值、PLT、PLR依次升高,LYM依次降低(P<0.05)。死亡組年齡、APACHEⅡ評分、SOFA評分、PLR、ALT/ALP比值、血乳酸水平高于存活組(P<0.05),2組肝損傷程度比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。多因素Logistic回歸分析結(jié)果顯示,高SOFA評分、高PLR、高ALT/ALP比值、中重度肝損傷是膿毒癥休克并發(fā)肝損傷患者死亡的危險因素。ROC曲線分析顯示,聯(lián)合ALT/ALP比值、PLR的預(yù)測效能[曲線下面積為0.915(95%CI:0.849~0.959),敏感度、特異度分別為94.20%、89.36%]優(yōu)于單一預(yù)測(P<0.05)。結(jié)論 高ALT/ALP比值和PLR與肝損傷程度加重及預(yù)后不良有關(guān),可作為預(yù)測膿毒癥休克并發(fā)肝損傷患者預(yù)后的潛在標(biāo)志物。

The relationship between ALT/ALP ratio, PLR and hepatic injury in elderly patients with septic shock

Abstract: Objective To investigate the prognostic value of alanine aminotransferase (ALT)/alkaline phosphatase (ALP) ratio, platelet (PLT) count/lymphocyte (LYM) ratio (PLR) in elderly patients with septic shock complicated with liver injury. Methods The clinical data of 116 patients with septic shock complicated with liver injury were retrospectively analyzed. According to the degree of liver injury, patients were divided into the mild group (39 cases), the moderate group (47 cases) and the severe group (30 cases). According to the survival of patients 28 days after admission, patients were divided into the death group (69 cases) and the survival group (47 cases). Acute physiological score, chronic healthⅡ(APACHE Ⅱ) score and sequential organ failure (SOFA) score were collected. PLT and LYM in peripheral blood were detected, and PLR was calculated. ALT and ALP were measured and ALT/ALP ratio was calculated. White blood cell count, blood lactic acid, C-reactive protein and other laboratory indicators were detected. Multivariate Logistic regression was used to analyze prognostic factors of patients with septic shock complicated with liver injury. The predictive values of ALT/ALP ratio and PLR on the prognosis of patients with septic shock complicated with liver injury were analyzed by receiver operating characteristic (ROC) curve. Results Serum ALT and ALP levels, ALT/ALP ratio, PLT and PLR were increased successively in the mild group, the moderate group and the severe group, while LYM was decreased successively in the three groups (P<0.05). The age, APACHE Ⅱ score, SOFA score, PLR, ALT/ALP ratio and blood lactic acid level were higher in the death group than those in the survival group (P<0.05). There was no significant difference in liver injury degree between the two groups (P<0.05). Multivariate Logistic regression analysis showed that high SOFA score, high PLR, high ALT/ALP ratio and moderate to severe liver injury were risk factors for death in patients with septic shock complicated with liver injury. ROC curve analysis showed that the combined ALT/ALP ratio and PLR prediction efficiency [area under the curve 0.915 (95%CI: 0.849-0.959), sensitivity and specificity 94.20% and 89.36%, respectively] was superior to single prediction (P<0.05). Conclusion Increased ALT/ALP ratio and PLR are associated with aggravation of liver injury and poor prognosis in patients with septic shock. ALT/ALP ratio and PLR can be used as prognostic markers.

膿毒癥是一種機(jī)體對感染產(chǎn)生的免疫失調(diào)而導(dǎo)致的危及生命的器官功能障礙[1]。肝臟在代謝和免疫穩(wěn)態(tài)中起著核心作用,肝損傷是膿毒癥休克患者常見的并發(fā)癥之一,亦是膿毒癥患者死亡的危險因素之一[2-3]。因此,探討膿毒癥休克致急性肝損傷的潛在因素和標(biāo)志物,對改善肝損傷、恢復(fù)肝功能以及降低死亡率至關(guān)重要。丙氨酸轉(zhuǎn)氨酶(alanine aminotransferase,ALT)、堿性磷酸酶(alkaline phosphatase,ALP)是反映肝臟損害的敏感指標(biāo),肝損傷可引起血清ALT、ALP水平升高[4]。ALT/ALP比值綜合ALT、ALP兩項(xiàng)指標(biāo),與肝損傷病理特征、病情變化的關(guān)系更為密切[5]。血小板計(jì)數(shù)(PLT)與淋巴細(xì)胞計(jì)數(shù)(LYM)比值(platelet lymphocyte ratio,PLR)是炎癥反應(yīng)標(biāo)志物之一,可反映膿毒癥全身炎癥反應(yīng)程度,高PLR是膿毒癥患者死亡的危險因素[6]。但目前有關(guān)ALT/ALP比值、PLR與膿毒癥休克并發(fā)肝損傷病情評估和預(yù)后的研究尚少見。本研究旨在探討ALT/ALP比值、PLR與膿毒癥休克并發(fā)肝損傷的關(guān)系,為其臨床診治和預(yù)后評估提供參考。
1 資料與方法
1.1 臨床資料 選擇2019年1月—2024年1月我院急診醫(yī)學(xué)科收治的膿毒癥休克并發(fā)肝損傷患者116例,其中男67例,女49例,年齡60~75歲,平均(65.56±4.92)歲;55例(47.41%)有吸煙史,47例(40.52%)有飲酒史;肺部感染43例(37.07%)、腹腔感染29例(25.00%)、皮膚及軟組織感染15例(12.93%)、泌尿系統(tǒng)感染29例(25.00%);高血壓54例(46.55%),高脂血癥49例(42.24%),糖尿病39例(33.62%)。納入標(biāo)準(zhǔn):(1)符合膿毒癥-3.0定義及膿毒癥休克診斷標(biāo)準(zhǔn)[7]。(2)經(jīng)臨床診斷為膿毒癥相關(guān)肝損傷,膽紅素≥34.2 μmol/L(2 mg/dL),凝血功能障礙國際標(biāo)準(zhǔn)化比值(INR)>1.5[8]。(3)年齡60周歲及以上,臨床病歷完整。排除標(biāo)準(zhǔn):(1)入院24 h內(nèi)出院或死亡患者。(2)慢性肝病,如慢性肝炎、肝硬化、肝惡性腫瘤、藥物性肝損傷、酒精性肝損傷。(3)肝手術(shù)史。(4)阻塞性黃疸。(5)合并其他器官損傷。(6)ALT、ALP、PLT、LYM實(shí)驗(yàn)數(shù)據(jù)缺失者。根據(jù)肝損傷程度分為輕度組(1<ALT/ALT參考值上限(ULN)<3或1<TBIL/TBILULN≤2,39例)、中度組(3≤ALT/ALTULN<5或2<TBIL/TBILULN≤3,47例)、重度組(ALT/ALTULN≥5或TBIL/TBILULN>3,30例)[8]。本研究經(jīng)我院倫理委員會批準(zhǔn)(20191108002)。
1.2 臨床結(jié)局追蹤和資料收集 統(tǒng)計(jì)膿毒癥休克并發(fā)肝損傷患者入院28 d存活情況,將患者分為死亡組(69例)和存活組(47例)。收集患者年齡,性別,吸煙史,飲酒史,基礎(chǔ)疾?。ㄌ悄虿 ⒏哐獕?、高脂血癥),感染部位(肺、腹腔、皮膚及軟組織、泌尿系統(tǒng)),急性生理與慢性健康Ⅱ(acute physiology and chronic health Ⅱ,APACHEⅡ)評分[9],序貫器官衰竭(sequential organ failure score,SOFA)評分[10]。
1.3 實(shí)驗(yàn)室檢查 所有患者入組后24 h內(nèi)均采集晨起肘靜脈血7 mL,完善實(shí)驗(yàn)室檢查。2 mL血標(biāo)本注入EDTA抗凝試管混勻,采用Sysmex XN-9000全自動血液分析儀(日本Sysmex公司)檢測外周血中白細(xì)胞計(jì)數(shù)(WBC)、PLT、LYM,計(jì)算PLR。3 mL血標(biāo)本注入干燥試管,室溫靜置取上層液,離心獲得血清樣本,采用C6000全自動生化分析儀(瑞士羅氏公司)檢測ALT、ALP,計(jì)算ALT/ALP比值及血清尿酸、血肌酐、尿素氮、TBIL、血乳酸水平。靜脈血2 mL注入血凝管混勻后,采用9600全自動凝血儀(日本Sysmex公司)測定D-二聚體、凝血酶原時間、INR。GBI-MAP 800全自動化學(xué)發(fā)光免疫分析儀(武漢華大生物科技有限公司)測定C反應(yīng)蛋白(CRP)、降鈣素原(PCT)。
1.4 統(tǒng)計(jì)學(xué)方法 采用SPSS 25.0軟件進(jìn)行數(shù)據(jù)分析。計(jì)量資料以均數(shù)±標(biāo)準(zhǔn)差([[x] ±s])表示,多組間比較采用單因素方差分析,組間多重比較行LSD-t檢驗(yàn);2組間比較采用獨(dú)立樣本t檢驗(yàn)。計(jì)數(shù)資料以例(%)或例表示,組間比較采用c2檢驗(yàn)。采用多因素Logistic回歸分析膿毒癥休克并發(fā)肝損傷患者預(yù)后的影響因素。采用受試者工作特征(ROC)曲線分析ALT/ALP比值、PLR對膿毒癥休克并發(fā)肝損傷患者預(yù)后的預(yù)測價值,DeLong檢驗(yàn)(MedCalc軟件)比較曲線下面積(AUC)。P<0.05為差異有統(tǒng)計(jì)學(xué)意義。
2 結(jié)果
2.1 不同肝損傷程度組ALT/ALP比值、PLR比" "較 輕度組、中度組和重度組血清ALT、ALP、ALT/ALP比值、PLT、PLR依次升高,LYM依次降低,組間多重比較差異均有統(tǒng)計(jì)學(xué)意義(P<0.05),見表1。
2.2 不同預(yù)后組臨床資料比較 死亡組年齡、APACHEⅡ評分、SOFA評分、PLR、ALT/ALP比值、血乳酸水平高于存活組(P<0.05),2組肝損傷程度比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05),其余指標(biāo)差異均無統(tǒng)計(jì)學(xué)意義(P>0.05),見表2。
2.3 膿毒癥休克并發(fā)肝損傷患者預(yù)后的影響因素分析 以膿毒癥休克并發(fā)肝損傷患者預(yù)后(存活=0,死亡=1)為因變量,以年齡、APACHEⅡ評分、SOFA評分、PLR、ALT/ALP比值、血乳酸、肝損傷程度(輕度=0,中重度=1)為自變量,進(jìn)行向后逐步法多因素Logistic回歸分析。結(jié)果顯示,高SOFA評分、高PLR、高ALT/ALP比值、中重度肝損傷是膿毒癥休克并發(fā)肝損傷患者死亡的危險因素(P<0.05),見表3。
2.4 ALT/ALP比值、PLR對膿毒癥休克并發(fā)肝損傷患者預(yù)后的預(yù)測價值 ALT/ALP比值,PLR預(yù)測膿毒癥休克并發(fā)肝損傷患者預(yù)后的AUC分別為0.765、0.734。聯(lián)合ALT/ALP比值、PLR預(yù)測膿毒癥休克并發(fā)肝損傷患者預(yù)后的AUC為0.915,高于ALT/ALP比值、PLR單一預(yù)測(Z分別為3.155、3.794,P<0.05),見表4、圖1。
3 討論
膿毒癥是宿主對感染反應(yīng)失調(diào)引起的危及生命的器官功能障礙,其在世界范圍內(nèi)的發(fā)病率和死亡率均較高[11]。肝臟對膿毒癥的免疫和代謝平衡至關(guān)重要,其可吞噬侵襲性致病菌,產(chǎn)生急性期蛋白和細(xì)胞因子,進(jìn)而激發(fā)適應(yīng)性炎癥反應(yīng)并調(diào)節(jié)免疫防御功能。然而,在全身性感染過程中,膿毒癥引起的全身炎癥反應(yīng)可損傷肝細(xì)胞,導(dǎo)致肝臟器官功能障礙。受損的肝細(xì)胞釋放損傷相關(guān)的分子模式,進(jìn)一步加劇全身炎癥反應(yīng),嚴(yán)重時甚至可導(dǎo)致患者死亡[12]。因此,探索與膿毒癥肝損傷預(yù)后相關(guān)的高效標(biāo)志物至關(guān)重要,可為臨床醫(yī)生提供關(guān)鍵信息以優(yōu)化治療策略,改善患者預(yù)后。
ALT是一種特異性肝酶,存在于肝細(xì)胞的細(xì)胞質(zhì)中。當(dāng)肝細(xì)胞損傷時,細(xì)胞膜通透性增加,導(dǎo)致ALT釋放至外周血中,引起血清ALT水平升高[13]。ALP是經(jīng)肝臟向膽外排出的一種酶,廣泛分布于人體的多種組織中,如肝臟、骨骼、腸、腎和胎盤等。在各類肝臟疾?。òǜ窝?、脂肪變性、肝硬化、藥物性肝損傷和肝癌)中,ALP水平可出現(xiàn)輕度至中度的升高;此外,高水平ALP可通過去除脂多糖脂質(zhì)A結(jié)構(gòu)中的末端磷酸基團(tuán)來減輕其誘導(dǎo)的毒性,這有助于預(yù)防或治療膿毒癥、酒精性肝損傷、急性腎損傷等疾?。?4]。本研究結(jié)果顯示,肝損傷輕度組、中度組和重度組患者血清ALT、ALP水平依次升高,與既往研究結(jié)果[15-16]一致,提示ALT、ALP水平可反映膿毒癥所致肝損傷嚴(yán)重程度。然而,本研究結(jié)果顯示,輕度組、中度組和重度組ALT/ALP比值依次升高,死亡組ALT/ALP比值高于存活組;且高ALT/ALP比值是膿毒癥休克并發(fā)肝損傷患者死亡的危險因素;ALT/ALP比值預(yù)測膿毒癥休克并發(fā)肝損傷患者預(yù)后的AUC為0.765,提示ALT/ALP比值對膿毒癥休克并發(fā)肝損傷患者預(yù)后的預(yù)測效能更高,其與預(yù)后關(guān)系更為密切。ALT和ALP檢測簡便且成本低廉,ALT/ALP比值易于計(jì)算,具有較廣闊的應(yīng)用前景。
PLR是反映血栓形成和炎癥狀態(tài)的綜合性指標(biāo)。血小板參與膿毒癥的病理和生理過程,并在器官功能障礙中發(fā)揮關(guān)鍵作用。膿毒癥引起的炎癥和內(nèi)皮細(xì)胞損傷可誘導(dǎo)血小板活化,活化的血小板進(jìn)一步加劇了凝血障礙和全身炎癥反應(yīng)[17]。淋巴細(xì)胞作為機(jī)體免疫功能的標(biāo)志物,在膿毒癥中因免疫抑制而減少,導(dǎo)致更嚴(yán)重的感染,并增加了膿毒癥相關(guān)死亡風(fēng)險[18]。Kosekli等[19]指出肝纖維化患者PLR高于健康對照組,且在晚期肝纖維化患者中PLR更高。廖國豪等[20]研究表明膽源性急性胰腺炎患者PLR升高與并發(fā)肝損傷有關(guān)。胡瓊英等[21]研究顯示PLR升高與肝細(xì)胞癌患者預(yù)后不良有關(guān),可作為預(yù)后評估的指標(biāo)之一。本研究結(jié)果顯示,輕度組、中度組和重度組PLR依次升高,死亡組PLR高于存活組;高PLR是膿毒癥休克并發(fā)肝損傷患者死亡的危險因素。分析膿毒癥休克并發(fā)肝損傷患者PLR升高的原因?yàn)椋貉“蹇芍苯幼饔糜赑-選擇素/P-選擇素糖蛋白配體-1(PSGL-1)和CD40/CD40配體信號通路,促使趨化因子和細(xì)胞因子分泌,增加血管通透性,促進(jìn)白細(xì)胞募集,誘導(dǎo)血管炎癥和內(nèi)皮功能障礙,從而導(dǎo)致肝臟損傷[22]。同時,淋巴細(xì)胞減少會加重免疫抑制和機(jī)體炎癥反應(yīng),促使炎性因子在肝細(xì)胞中浸潤,進(jìn)一步加重肝損傷。本研究結(jié)果顯示,PLR預(yù)測膿毒癥休克并發(fā)肝損傷患者預(yù)后的AUC高于PLT、LYM,分析其原因是,PLT和LYM易受感染、炎癥性疾病、血液系統(tǒng)疾病、免疫抑制藥物的使用等多種因素的影響,而選用PLR更穩(wěn)定,結(jié)果更可靠。另外,本研究結(jié)果顯示,聯(lián)合ALT/ALP比值、PLR預(yù)測膿毒癥休克并發(fā)肝損傷患者預(yù)后的AUC高于ALT/ALP比值、PLR單一預(yù)測,提示ALT/ALP比值和PLR可作為預(yù)測膿毒癥休克并發(fā)肝損傷患者預(yù)后的潛在標(biāo)志物,且二者聯(lián)合檢測可提高預(yù)測效能。
綜上,膿毒癥休克并發(fā)肝損傷患者ALT/ALP比值和PLR均顯著增高,高ALT/ALP比值和PLR與肝損傷程度加重以及預(yù)后不良有關(guān),可作為預(yù)測膿毒癥休克并發(fā)肝損傷患者預(yù)后的潛在標(biāo)志物。
