[摘要] 目的 評估以臨床勝任力為導(dǎo)向的教學(xué)法在疼痛醫(yī)學(xué)臨床操作教學(xué)中的應(yīng)用效果。 方法 選擇2017年5月~2018年5月宣武醫(yī)院疼痛科40名進(jìn)修醫(yī)生為研究對象,采用隨機(jī)數(shù)字表法將其分為“以能力為基礎(chǔ)的教育”(CBE)教學(xué)組和傳統(tǒng)教學(xué)組,每組各20名進(jìn)修醫(yī)生。記錄教學(xué)考試成績、滿意度和教學(xué)實(shí)施效果。 結(jié)果 與傳統(tǒng)教學(xué)組比較,CBE教學(xué)組的技能考試成績和教學(xué)滿意度更高(P < 0.05),CBE教學(xué)組能明顯提高學(xué)習(xí)注意力、學(xué)習(xí)效率、學(xué)習(xí)積極性、理論聯(lián)系實(shí)際并靈活的能力、解決問題的能力、臨床工作能力和自主學(xué)習(xí)能力(P < 0.05)。結(jié)論 應(yīng)用臨床勝任力為導(dǎo)向的教學(xué)模式促進(jìn)了教學(xué)向“以學(xué)生為中心”轉(zhuǎn)變,有利于調(diào)動(dòng)進(jìn)修醫(yī)生學(xué)習(xí)的主動(dòng)性和積極性,有利于提高臨床操作學(xué)習(xí)的教學(xué)效果和學(xué)生的實(shí)際操作能力。

Application of clinical competency-oriented teaching method in clinical pracitcal pain medicine

[Abstract] Objective To evaluate the efficacy of clinical competency-oriented teaching method in clinical practical pain. Methods From May 2017 to March 2018, 40 trainee doctors in Xuanwu Hospital, Capital Medical University were selected as study object and randomly divided into competency based education(CBE) teaching group and traditional teaching group, with 20 trainee doctors in each group. Teaching examination results, satisfaction and teaching implementation effect were recorded. Results Compared with traditional teaching group, the teaching examination results and satisfaction score were higher in the CBE teaching group (P < 0.05), and the improvement of learning attention, learning efficiency, learning motivation, capacity of theory with practice, problem-solving capability, clinical competence and independent learning ability were more obviously in CBE teaching group (P < 0.05). Conclusion The clinical competency-oriented teaching method can promote the switch from being teacher-centered to student-centered, inspire the student′s initiative and enthusiasm and improve the clinical teaching effect and practical ability.

醫(yī)師臨床勝任力,是在人力資源管理學(xué)中“勝任力”的概念演變和發(fā)展而來,并由美國畢業(yè)后醫(yī)學(xué)教育認(rèn)證委員會(huì)(ACGME)、加拿大皇家內(nèi)科及外科醫(yī)師協(xié)會(huì)(RCPSC)等機(jī)構(gòu)制定了相應(yīng)的評價(jià)指標(biāo)體系和指南[1]。為了提高學(xué)生綜合能力、勝任崗位工作的需要,提出了“以能力為基礎(chǔ)的教育”(competency based education,CBE)理論[2]。其教學(xué)特點(diǎn)包括:①與傳統(tǒng)純理論知識(shí)教學(xué)相悖,在技能學(xué)習(xí)中掌握專業(yè)知識(shí);②課程設(shè)計(jì)以實(shí)用性、可操性為主;③教師扮演協(xié)助角色,學(xué)生在實(shí)踐中扮演主體角色,增加實(shí)踐比重;④考評手段以解決實(shí)際問題能力為主。目前,臨床勝任力為導(dǎo)向的CBE教學(xué)法被成功用于醫(yī)學(xué)專業(yè)的相關(guān)領(lǐng)域[3-9]。
宣武醫(yī)院疼痛科每年接受近80名長期進(jìn)修醫(yī)生,承擔(dān)著大量疼痛專科醫(yī)師培養(yǎng)的任務(wù),致力于疼痛學(xué)的教學(xué)改革深入開展[10-12]。目前,進(jìn)修醫(yī)生多為其他臨床醫(yī)學(xué)專業(yè)轉(zhuǎn)型而來,雖已經(jīng)具備醫(yī)學(xué)的基礎(chǔ)理論、基礎(chǔ)知識(shí)和基本技能,但無系統(tǒng)的疼痛醫(yī)學(xué)理論和實(shí)踐學(xué)習(xí),缺乏疼痛醫(yī)學(xué)臨床思維,對患者整體性處理欠佳。因此,如何在短期內(nèi)建立完整的疼痛醫(yī)學(xué)臨床思維、勝任疼痛科臨床工作,高效實(shí)現(xiàn)由其他專業(yè)醫(yī)師向疼痛科專科醫(yī)師的轉(zhuǎn)變,成為培養(yǎng)過程中迫切需要解決的問題。首都醫(yī)科大學(xué)宣武醫(yī)院(以下簡稱“我院”)疼痛科在教學(xué)中注重進(jìn)修醫(yī)師臨床思維的轉(zhuǎn)變、實(shí)戰(zhàn)能力的掌握,采用以臨床勝任力為導(dǎo)向的教學(xué)法在疼痛醫(yī)學(xué)臨床操作教學(xué)中取得了一定的教學(xué)成效,現(xiàn)總結(jié)如下:
