【摘要】 目的:探究袋鼠式護(hù)理聯(lián)合音樂療法在新生兒缺氧缺血性腦病護(hù)理中的應(yīng)用效果。方法:選取2017年1月-2019年1月筆者所在醫(yī)院新生兒監(jiān)護(hù)室科收治的74例缺氧缺血性腦病患兒作為研究對象,隨機(jī)分為兩組。普通護(hù)理組(B組,35例)采用常規(guī)護(hù)理,綜合護(hù)理組(A組,39例)在常規(guī)護(hù)理基礎(chǔ)上采用袋鼠式護(hù)理聯(lián)合音樂療法,比較兩組護(hù)理效果。結(jié)果:護(hù)理前兩組NBNA量表評分比較差異無統(tǒng)計學(xué)意義(P>0.05),出院時A組患兒各項評分均高于B組,差異均有統(tǒng)計學(xué)意義(P<0.05);A組神經(jīng)系統(tǒng)損害發(fā)生率低于B組,并發(fā)癥發(fā)生率低于B組,ICU住院時間短于B組,患兒家長護(hù)理滿意度評分高于B組,差異均有統(tǒng)計學(xué)意義(P<0.05)。結(jié)論:新生兒缺氧缺血性腦病實施袋鼠式護(hù)理聯(lián)合音樂療法,可有效促進(jìn)患兒神經(jīng)行為的發(fā)育,且可保護(hù)患兒神經(jīng)系統(tǒng)功能,降低神經(jīng)系統(tǒng)損害及并發(fā)癥發(fā)生率,值得推薦。

[Abstract] Objective: To explore the effect of kangaroo nursing combined with music therapy in neonatal hypoxic-ischemic encephalopathy nursing. Method: From January 2017 to January 2019, 74 children with hypoxic-ischemic encephalopathy treated in the neonatal intensive care unit of the hospital in the hospital were selected as the subject of study, and randomly divided into two groups. The general nursing group (group B, n=35) was treated with routine nursing, and the comprehensive nursing group (group A, n=39) was treated with kangaroo nursing combined with music therapy on the basis of routine nursing. The nursing effects of the two groups were compared. Result: There was no significant difference between the two groups before nursing (P>0.05). At the time of discharge, the scores of the group A in group A were higher than that of group B, the differences were statistically significant (P<0.05). The incidence of nervous system damage in group A was lower than that in group B, the incidence of complications was lower than that in group B, the hospitalization time of ICU was shorter than that in group B, and the nursing satisfaction score of parents in group A was significantly higher than that in group B, the differences were statistically significant (P<0.05). Conclusion: The kangaroo nursing combined with music therapy of the neonatal hypoxic-ischemic encephalopathy can effectively promote the development of the neurobehavioral development of the child, and can protect the nervous system function of the child, reduce the damage of the nervous system and the complication rate, and is worth recommending.
新生兒缺氧缺血性腦病(hypoxie-ischemic encephalopathy,HIE)是指在患兒在圍產(chǎn)期大腦發(fā)生缺氧缺血性損害。HIE嚴(yán)重威脅著新生兒的生存率,存活新生兒后期面臨病殘的風(fēng)險極大。HIE患兒由于腦組織長時間和缺血缺氧后的產(chǎn)生的腦損傷狀態(tài)下容易出現(xiàn)全身神經(jīng)系統(tǒng)的后遺癥狀,給HIE患兒本身及家庭帶來長期困擾,增加了社會負(fù)擔(dān)[1-2]。目前,通過使用創(chuàng)新的護(hù)理方案以改善生長發(fā)育狀況是臨床護(hù)理工作的重點研究方向,HIE患兒正處于大腦器官發(fā)育尚未完全成熟階段,中樞神經(jīng)系統(tǒng)發(fā)育可塑性較強(qiáng),因此在該階段對其進(jìn)行合理有效的護(hù)理干預(yù)可在一定程度上改善新生兒腦發(fā)育及遠(yuǎn)期預(yù)后狀況[3-5]。本研究基于此,選取2017年1月-2019年1月筆者所在醫(yī)院新生兒監(jiān)護(hù)室科收治的74例缺血缺氧性腦病患兒作為研究對象,探究實行袋鼠式護(hù)理聯(lián)合音樂療法在HIE患兒中的應(yīng)用效果,具體如下。
綜上所述,缺氧缺血性腦病新生兒實施袋鼠式護(hù)理聯(lián)合音樂方法,可有效促進(jìn)患兒神經(jīng)行為的發(fā)育,且可保護(hù)患兒的神經(jīng)系統(tǒng)功能,降低神經(jīng)系統(tǒng)損害的發(fā)生率,值得推薦。
