糖尿病患者血漿中半乳糖凝集素-3水平的表達

目的 探討半乳糖凝集素-3(Galectin-3)在糖尿病患者血漿中的表達情況，以及與其他因素的相關(guān)性分析。方法 對30例糖尿病患者、22例非糖尿病患者(對照組)血漿中的Galectin-3進行測定，比較兩組間Galectin-3水平變化，分析Galectin-3與其他因素的相關(guān)性。結(jié)果 糖尿病患者血漿中的Galectin-3水平[(17.95±2.61) μg/L]顯著高于對照組[(15.59±2.39)μg/L]，差異有統(tǒng)計學意義(P<0.01)。Galectin-3與空腹血糖(FBG)有顯著地相關(guān)性(P<0.01)，與糖尿病病程(DD)、鈉尿肽(BNP)有明顯相關(guān)性(P<0.05)。結(jié)論 Galectin-3作為重要的炎癥調(diào)節(jié)因子，可能在糖尿病的發(fā)生發(fā)展中起著重要的調(diào)節(jié)作用。

近年研究發(fā)現(xiàn)，半乳糖凝集素(Galectin-3)是一種重要的炎癥、免疫、腫瘤調(diào)節(jié)因子，在心力衰竭的診斷、預后評估中具有重要價值[1-2]。2010年，F(xiàn)DA批準了Galectin-3的臨床應用，目前Galectin-3已成為歐美國家臨床診斷與評估心力衰竭患者近期預后的重要指標[3-4]。然而，作為重要的炎癥調(diào)節(jié)因子，Galectin-3在糖尿病及其并發(fā)癥中的作用研究還比較罕見。本文就Galectin-3在糖尿病患者血漿中的表達水平，以及與其相關(guān)的因素進行了初步探討。

1 對象與方法

1.1 研究對象 選取2013年1—5月在我院老年心血管科住院的患者52例，其中糖尿病患者30例為研究組，男28例，女2例，年齡48～87歲，平均年齡(65.3±9.7)歲；非糖尿病患者22例為對照組，男19例，女3例，年齡48～86歲，平均(61.6±12.1)歲。入選對象排除標準：心力衰竭患者、腫瘤患者、嚴重肝腎功能不全患者。兩組總膽固醇(TC)、低密度脂蛋白膽固醇(LDL-C)、肌酐(Cr)、尿素氮(BUN)和尿酸(BUA)水平，以及高血壓、冠心病患病率兩組間差異均無統(tǒng)計學意義(P>0.05)。

1.2 方法 Galectin-3檢測，入院次日清晨空腹采靜脈血4 mL于EDTA抗凝管中，在4℃狀態(tài)下離心3000 r/min ,20 min，分離血漿放置-80℃保存，擇日統(tǒng)一由北京海斯特醫(yī)學檢驗中心測定，試劑為BG medicine 公司的Galectin-3 Elisa檢測試劑盒。鈉尿肽(BNP)、各項生化指標由我院檢驗科測定。

1.3 統(tǒng)計學處理 采用SPSS 17.0統(tǒng)計軟件，計量資料組間比較用獨立樣本的t檢驗；計數(shù)資料采用χ2檢驗；相關(guān)性分析采用Pearson相關(guān)分析。P<0.05為差異有統(tǒng)計學意義。

2 結(jié)果

2.1 兩組患者血漿中Galectin-3水平的比較 與非糖尿病組比較，糖尿病組Galectin-3水平顯著增高，差異有統(tǒng)計學意義(P<0.01)；BNP水平兩組間差異無統(tǒng)計學意義(P>0.05)，見表1。

2.2 Galectin-3與部分臨床變量的相關(guān)性分析 見表2。

3 討論

糖尿病的發(fā)病機制被認為與機體的炎癥過程密切相關(guān)，已形成糖尿病的炎癥發(fā)病學說。國外研究提示，Galectin-3通過促進炎癥的發(fā)生，來加速代謝性疾病的發(fā)病機制[5-6]。因此，作為炎性反應重要介質(zhì)的Galectin-3，在糖尿病及其并發(fā)癥中的作用必然受到人們的關(guān)注[7]。Seferovic等[8]研究發(fā)現(xiàn)，糖尿病合并高血壓患者血漿中的Galectin-3水平顯著高于單純糖尿病或高血壓患者。Jin等[9]研究結(jié)果顯示，2型糖尿病伴血管并發(fā)癥患者血漿中的Galectin-3水平明顯高于無并發(fā)癥的糖尿病患者(P<0.05)，Galectin-3水平升高可增加2型糖尿病患者血管并發(fā)癥發(fā)生的風險；Galectin-3與糖尿病病程、糖化血紅蛋白有明顯的相關(guān)性(P<0.05)。作者認為，Galectin-3與糖尿病患者血管病的發(fā)病機制有關(guān)。目前，我們尚未見到研究糖尿病與非糖尿病患者之間Galectin-3水平變化及其作用的文獻。本文研究結(jié)果顯示，糖尿病患者血漿中的Galectin-3水平顯著高于非糖尿病者(P<0.01)，而同樣作為心力衰竭診斷標志物的BNP則無明顯變化。而且，經(jīng)Pearson相關(guān)性分析表明，Galectin-3水平與糖尿病患者的病程、FBG水平和BNP有顯著的相關(guān)性(P<0.01或P<0.05)。本文研究結(jié)果提示，Galectin-3不僅是一種心力衰竭的診斷標志物，而且還是一種炎癥調(diào)節(jié)因子參與炎癥的發(fā)生發(fā)展。FBG水平越高，糖尿病病程越長，血漿中的Galectin-3水平則表達的越高，提示炎癥程度越高，病情越嚴重，發(fā)生糖尿病并發(fā)癥的風險越大。

Expression of plasma galectin-3 in patients with diabetes mellitus

Objective To analyze the plasma galectin-3 level in patients with diabetes mellitus and its relevant risk factors.Methods Thirty patients with diabetes mellitus served as study group and twenty-two individuals without diabetes mellitus served as control group were enrolled in this study.The levels of plasma Galectin-3 were measured and compared between the two groups.The correlation between plasma galectin-3 and other risk factors were analyzed.Results Levels of plasma Galectin-3 was (17.95±2.61)μg/L in study group,and (15.59±2.39)μg/L in control group.Difference in levels of plasma galectin-3 between the two groups was significant(P<0.01).Pearson analysis showed that plasma galectin-3 had significant correlation with fasting blood glucose,duration of diabetes,and B-type natriuretic peptide(P<0.01 orP<0.05).Conclusion As an important regulation factor of inflammation,galectin-3 may play a role in the pathogenesis of diabetes mellitus.
