Practice of counterpart assistance and special treatment for serious diseases in tertiary hospitals

[Abstract] In order to further consolidate the counterpart work of tertiary hospitals to assist county-level hospitals in poverty-stricken counties， deepen the reform of medical and health system， optimize the allocation of medical resources， enhance the capacity of primary medical and health services， and better meet the medical and health service needs of the masses， we strived to achieve the goals of "sending a team， leading a hospital（health center）， serving a corner of masses and training a group of talents"， and implemented measures such as "project assistance， team assistance and overall assistance" in Kashgar area. We made plans for the treatment of serious diseases and construct a medical community so as to significantly improve the diagnosis and treatment capacity of the assisted hospitals. We effectively promoted the development of medical community work， and it is a good way to rapidly improve the service level of county-level hospitals in poverty-stricken counties.

黨的十八大以來，以習(xí)近平同志為核心的黨中央堅持以人民為中心的發(fā)展思想，作出了到2020年現(xiàn)行標(biāo)準(zhǔn)下農(nóng)村貧困人口實現(xiàn)脫貧、貧困縣全部摘帽、解決區(qū)域性整體貧困、堅決打贏脫貧攻堅戰(zhàn)的戰(zhàn)略部署。根據(jù)國家衛(wèi)生健康委辦公廳《關(guān)于開展三級醫(yī)院對口幫扶貧困縣縣級醫(yī)院和農(nóng)村貧困人口大病專項救治調(diào)查工作的通知》要求，推動三級醫(yī)院對口幫扶貧困縣縣級醫(yī)院見真效。深入貫徹落實習(xí)近平總書記在6月2日主持召開專家學(xué)者座談會上的重要講話精神，把疫情暴露出基層醫(yī)療救治體系方面存在的薄弱環(huán)節(jié)和突出問題，作為統(tǒng)籌推進(jìn)三級醫(yī)院對口幫扶的一項重要任務(wù)，對照縣級醫(yī)院服務(wù)能力基本標(biāo)準(zhǔn)、推薦標(biāo)準(zhǔn)和重大疾病救治短板弱項，統(tǒng)籌各方支援力量，向外轉(zhuǎn)率高的病種所涉及的臨床學(xué)科和平臺學(xué)科建設(shè)發(fā)力，使常見病、多發(fā)病、部分危急重癥能夠在縣域內(nèi)得到有效救治。按照“縣強、村活、上下聯(lián)、信息通”的要求，加快緊密型縣域醫(yī)共體建設(shè)，實現(xiàn)“90%大病不出縣”的幫扶目標(biāo)。加強縣域內(nèi)遠(yuǎn)程醫(yī)療體系建設(shè)，充分發(fā)揮“互聯(lián)網(wǎng)+醫(yī)療”在擴大優(yōu)質(zhì)醫(yī)療資源供給方面的優(yōu)勢，實現(xiàn)縣級醫(yī)院與鄉(xiāng)村（社區(qū)）醫(yī)療衛(wèi)生機構(gòu)遠(yuǎn)程醫(yī)療100%全覆蓋的任務(wù)目標(biāo)。

加強貧困地區(qū)醫(yī)療衛(wèi)生事業(yè)建設(shè)，全面改善貧困地區(qū)醫(yī)療衛(wèi)生機構(gòu)設(shè)施條件，提升縣域地區(qū)醫(yī)療衛(wèi)生服務(wù)能力，是解決貧困群眾看病就醫(yī)問題，有效緩解因病致貧、因病返貧的重要舉措，是深化健康扶貧的重要步驟[1]。

根據(jù)《關(guān)于進(jìn)一步強化三級醫(yī)院對口幫扶支援貧困縣縣級醫(yī)院工作的通知》[2]，為認(rèn)真貫徹落實黨中央、國務(wù)院解決“兩不愁三保障”突出問題決策部署和第七次全國對口支援新疆工作會議精神，以解決基本醫(yī)療有保障突出問題為主攻方向，進(jìn)一步夯實三級醫(yī)院對口幫扶貧困縣縣級醫(yī)院工作，葉城縣進(jìn)一步強化了三級醫(yī)院對口幫扶支援貧困縣縣級醫(yī)院、大病專項救治、醫(yī)共體建設(shè)等工作[3]。

1 對口幫扶工作實施情況與成果

2015年以來喀什地區(qū)第一、第二人民醫(yī)院21名專家的援建，以及解放軍986醫(yī)院48名專家的短期援建，分別援建了縣人民醫(yī)院產(chǎn)科、神經(jīng)內(nèi)科、消化腫瘤科、心內(nèi)科、神經(jīng)外科、普外科、代謝內(nèi)分泌科、骨二科、新生兒科、檢驗科、護(hù)理部、院感、醫(yī)務(wù)科等?？哲?86醫(yī)院及喀什第二人民醫(yī)院發(fā)揮醫(yī)療、科研、人才優(yōu)勢，通過“項目幫帶、團(tuán)隊幫帶、整體幫帶”，有力提升了當(dāng)?shù)蒯t(yī)院整體水平。
