[摘要]目的 探討血常規(guī)檢查和血清鐵蛋白檢查在缺鐵性貧血感染疾病中的診斷價值。方法 選取2018年2月~2019年4月我院收治且疑似缺鐵性貧血感染的患者68例作為研究對象,所有患者均接受血常規(guī)檢查和血清鐵蛋白檢查。比較兩種檢查方法的靈敏度、特異性和準確率。結果 68例疑似缺鐵性貧血感染的患者中,病理檢查確診為缺鐵性貧血感染的患者50例,非缺鐵性貧血感染的患者18例;血清鐵蛋白檢查缺鐵性貧血感染的患者41例,陰性27例;血常規(guī)檢查缺鐵性貧血感染的患者46例,陰性22例。兩種檢查方法的靈敏度、特異性和準確率比較,差異無統(tǒng)計學意義(P>0.05)。結論 缺鐵性貧血感染患者行血常規(guī)檢查和血清鐵蛋白檢查,其靈敏度、特異性和準確率相當,臨床應根據(jù)患者的實際情況選擇檢查方法。

Diagnostic value of routine blood examination and serum ferritin examination in iron deficiency anemia infection

[Abstract] Objective To explore the diagnostic value of routine blood examination and serum ferritin examination in iron deficiency anemia infection. Methods Sixty-eight patients suspected with iron deficiency anemia infection in our hospital from February 2018 to April 2019 were selected as the study objects, all patients received routine blood tests and serum ferritin tests. The sensitivity, specificity and accuracy of the two methods were compared. Results Among 68 patients suspected with iron deficiency anemia infection, 50 cases were diagnosed as iron deficiency anemia infection by pathological examination, and 18 cases were non iron deficiency anemia infection. 41 patients were diagnosed as iron deficiency anemia infection by serum ferritin, and 27 cases were detected in negative. 46 patients were diagnosed as iron deficiency anemia infection by blood routine examination, and 22 cases were detected in negative, there were no significant differences in sensitivity, specificity and accuracy between the two methods (P>0.05). Conclusion The sensitivity, specificity and accuracy of routine blood test and serum ferritin test in patients with iron deficiency anemia infection are similar. The clinical examination method should be selected according to the actual situation of patients.

當人體對鐵的需求以及供應失衡,將造成鐵在體內清空,發(fā)生缺鐵性貧血的疾病[1],為貧血疾病中最常見的一種,臨床可單獨出現(xiàn),也可伴隨其他疾病發(fā)生。隨著病情發(fā)展可見機體血紅蛋白(Hb)發(fā)生缺陷,體內的紅細胞生成受到感染,引起低色素性貧血??煽康脑\斷方法是保證疾病治療效果的關鍵所在,臨床一般將Hb及血清鐵蛋白電泳測定法對貧血進行鑒定,但上述方法的檢查所需時間較長。國內外大量研究顯示,血常規(guī)檢測可快速診斷缺鐵性貧血,有利于病癥的診斷和后期診治,其靈敏度、特異性以及準確性相對較高[2],同時平均紅細胞體積(MCV)和紅細胞體積分布寬度(RDW)的異常變化,還可鑒別貧血類型,與地中海貧血相區(qū)別,便于下一步治療方案的制訂與順利實施。缺鐵性貧血與地中海貧血均為小細胞低色素性貧血,血常規(guī)檢測具有操作簡單、準確性高等優(yōu)勢。本研究旨在探討血常規(guī)檢查和血清鐵蛋白檢查在缺鐵性貧血感染疾病中的診斷價值,現(xiàn)報道如下。
