Yet Chinas food industry is a typical description of “bad money driving out the good”.     With hospitals and nursing homes tending to thousands of patients every year,accidents can—and do—happen. These incidents,whether they are through carelessness or otherwise, can leave patients feeling powerless. Thats not the case.

    “There is growing public awareness. People are feeling they have more rights and they have tools in hand to make a complaint,” said Ralph Montano, spokesman for the California Department of Public Health, which regulates hospitals and longterm care facilities in the state.

    That department received more than 6,000 complaints about hospitals in 2007; in the most recent year statistics are available. The complaints can be about mixedup lab results, medicine errors, foreign objects left in a patient during surgery or a host of other topic.

    Similarly, the California Department of Aging received 43,000 nursing home complaints in 2009. Some said patient abuse or neglect of patients; others reported missing items. And some commented on the quality of the food.

    But finding the channels through them to put forward a complaint can be tiring and time consumption. Many consumers simply dont bother, and some become lost in the system. Whether the complaint is against a hospital or a longterm care facility, the process is similar—and many people can help, including the facilitys staff, insurance company representatives and state regulators.

    If you want to make a complaint while in the hospital, Patti Harvey, vice president of quality and patient care services for Kaiser Permanente in Southern California, recommends talking with the bedside nurse. If that doesnt work, you can talk with other people higher in the chain of command, up to the hospital administrator. If the problem isnt still taken care of—say you disagree with your treatment plan or have a problem with your doctor—member service offices at each hospital can help address your concerns.

    The advent of warm weather here in Maine calls for a seasonal ritual that, for me, puts an end to winter—fetching the wooden screen door.

    Note that I wrote “wooden”. The aluminum models just wont do it—they close too neatly and keep their perfect form for decades. In the 1960s, a time of imperfection, there was no perfect house, no perfect car, no perfect kids or parents. Why should a screen door be an exception?

    I grew up with my siblings in a workingclass neighborhood in New Jersey. When the warm weather arrived, my dad would pull out the wooden screen door and install it over the back door, which could then be left wide open, admitting a refreshing breeze (we had no air conditioning).

    My father was a Mr. Fixit, so keeping the screen door serviceable was one of his hobbies. Every few years he gave it a fresh coat of paint and fixed it a bit. I distinctly remember him putting the last screw in the last hinge (鉸鏈), and swinging the door shut with a “crack!”

    A good, wooden screen door, unsightly as it was, had an invaluable function in the age of the stayathome mom: It alerted her to the coming and going of the kids. My siblings, and my friends ran in and out of the house, tearing the screen door open and letting it slap shut behind us. A hundred times a day. It was all good, and my mom never complained about the noise, because that was the purpose of a wooden screen door—to slam shut and thereby announce that her children were within earshot.

    Several years ago, in a fit of nostalgia (懷舊), I went shopping for a wooden screen door. I was disappointed in the choices available. They looked a bit too solid, too well made. But I found one online, and within a week it was delivered to my doorstep.

    The firm had sent me one with the wrong dimensions, so I asked my carpenter to make the necessary adjustments. Ozzie worked away at it for a couple of hours until he got it to sit neatly in its frame. I gave it a test: I pulled it open and let go. It closed in a lazy fashion. “Not good,” I pronounced. “Please remove the automatic door closer and adjust the door so it swings shut with a good crack.”

    Ozzie went about his work and a short while later the task was done. The door was uneven in its frame, and the screen no longer lay flat.     British children used to play conkers (板栗游戲) in the autumn when the horsechestnut trees started to drop their shiny brown nuts. They would select a suitable chestnut, drill a hole in it and thread it onto a string, then swing their conker at that of an opponent until one of them broke. But the game has fallen out of favour. Children spend less time outdoors and rarely have access to chestnut trees. Besides, many schools have banned conkers games, worried that they might cause injuries or nut allergies.

    That sort of riskaverseness (規(guī)避風險) now spreads through every aspect of childhood. Playgrounds have all the excitement designed out of them to make them safe. Many governments, particularly in societies such as America, have tightened up their rules, requiring parents to supervise (監(jiān)管) young children far more closely than in the past. Frank Furedi of the University of Kent, a critic on modern parenting, argues that allowing children to play unsupervised or leaving them at home alone is increasingly described as a symptom of irresponsible parenting.

    In part, such increased caution is a response to the huge wave of changes. Largescale urbanization, smaller and more mobile families, the move of women into the labor market and the digitization of many aspects of life have unavoidably changed the way that people bring up their children. There is little chance that any of these trends will be changed, so todays more intensive (精細化的) parenting style is likely to go on.

    Such parenting practices now embraced by wealthy parents in many parts of the rich world, particularly in America, go far beyond an adjustment to changes in external conditions. They mean a strong bid to ensure that the advantages enjoyed by the parents generation are passed on to their children. Since success in life now turns mainly on education, such parents will do their best to provide their children with the schooling, the character training and the social skills that will secure access to the best universities and later the most attractive jobs.

    To some extent that has always been the case . But there are more such parents now, and they are competing with each other for what economists call positional goods. This competition starts even before the children are born. The wealthy classes will take their time to select a suitable spouse and get married, and will start a familyonly when they feel ready for it.

    Children from less advantaged backgrounds, by contrast, often appear before their parents are ready for them. In America 60% of births to single women under 30 are unplanned, and over 40% of children are born outside marriage. The result, certainly in America, has been to widen already massive social inequalities yet further.

    All the evidence suggests that children from poorer backgrounds are at a disadvantage almost as soon as they are born. By the age of five or six they are far less “schoolready” than their betteroff peers, so any attempts to help them catch up have to start long before they get to school. America has had some success with various schemes involving regular home visits by nurses or social workers to lowincome families with new babies. It also has long experience with programmes for young children from poor families that combine support for parents with goodquality child care. Such programmes do seem to make a difference. Without extra effort, children from lowincome families in most countries are much less likely than their betteroff peers to attend preschool education, even though they are more likely to benefit from it. And data from the OECDs PISA programme suggest that children need at least two years of preschool education to perform at their best when they are 15.

    So the most promising way to ensure greater equality may be to make earlyyears education and care for more widely available and more affordable, as it is in the Nordics. Some governments are already rethinking their educational priorities, shifting some of their spending to the early years.

    Most rich countries decided more than a century ago that free compulsory education for all children was a worthwhile investment for society. There is now an argument for starting preschool education earlier, as some countries have already done. In the face of crushing new inequalities, a modern version of that approach is worth trying.
