2016年世界衛(wèi)生組織(WHO)中樞神經(jīng)系統(tǒng)腫瘤分類將彌漫性中線膠質(zhì)瘤,H3 K27M?突變定義為彌漫性浸潤中線的高級別膠質(zhì)瘤,以星形膠質(zhì)細(xì)胞分化和組蛋白H3基因H3F3A或更為少見的HIST1H3BK27M?突變?yōu)橹饕卣鳌3 K27M?突變型彌漫性中線膠質(zhì)瘤好發(fā)于兒童,亦可見于成人,常見發(fā)生部位為腦干、丘腦和脊髓,通常預(yù)后不良。組織學(xué)形態(tài)觀察,典型病例呈現(xiàn)星形膠質(zhì)細(xì)胞形態(tài)特點,腫瘤細(xì)胞較單一、體積較?。▓D1a),亦可見多形性大細(xì)胞(圖1b)。此類腫瘤組織學(xué)形態(tài)譜系廣泛,包括少突膠質(zhì)細(xì)胞瘤樣、巨細(xì)胞、上皮樣細(xì)胞和橫紋肌樣細(xì)胞以及原始神經(jīng)外胚層腫瘤(PNET)樣區(qū)域、神經(jīng)氈樣島、毛細(xì)胞黏液樣特征、室管膜樣區(qū)、肉瘤樣區(qū)域、神經(jīng)節(jié)細(xì)胞分化以及多形性黃色瘤型星形細(xì)胞瘤(PXA)樣區(qū)域。約25%的腫瘤細(xì)胞可見壞死和核分裂象。免疫組織化學(xué)染色可見腫瘤細(xì)胞胞質(zhì)表達(dá)神經(jīng)細(xì)胞黏附分子?1(NCAM?1),胞質(zhì)和胞核表達(dá)S?100蛋白(S?100),胞核表達(dá)少突膠質(zhì)細(xì)胞轉(zhuǎn)錄因子?2(Olig?2),胞質(zhì)常表達(dá)微管相關(guān)蛋白?2(MAP?2)。H3 K27M?突變可以采用免疫組織化學(xué)染色,腫瘤細(xì)胞胞核呈陽性表達(dá)(圖2)。
