[摘要] 目的 研究胸腔鏡治療Ⅰ、Ⅱ期NSCLC(非小細(xì)胞肺癌)的臨床療效。 方法 將2012年1月~2013年12月在本院胸外科接受手術(shù)治療的Ⅰ、Ⅱ期NSCLC患者108例作為研究對(duì)象,按照隨機(jī)數(shù)字表法1∶1分成VATS組與開胸組,各54例。VATS組行VATS,開胸組行開胸肺癌根治術(shù)。觀察兩組手術(shù)時(shí)間、術(shù)中出血量、清掃淋巴結(jié)個(gè)數(shù)、術(shù)后引流時(shí)間等手術(shù)指標(biāo),術(shù)前及術(shù)后7 d CD3+、CD4+、CD8+等免疫細(xì)胞水平;手術(shù)前及術(shù)后7 d CRP、TNF-α、IL-6等炎性因子水平,3年內(nèi)生存率、并發(fā)癥、復(fù)發(fā)等指標(biāo)。 結(jié)果 VATS組手術(shù)時(shí)間、術(shù)中出血量、術(shù)后引流時(shí)間均低于開胸組,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。VATS組與開胸組清掃淋巴結(jié)個(gè)數(shù)無差異(P>0.05)。術(shù)后7 d VATS組CD3+、CD4+水平高于開胸組,CD8+水平低于開胸組,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。術(shù)后7 d VATS組CRP、TNF-α、IL-6水平低于開胸組,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。VATS組術(shù)后1、2、3年生存率(94.44%、81.48%、56.14%)高于開胸組(72.22%、57.41%、37.03%),差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。VATS組術(shù)后并發(fā)癥發(fā)病率(9.26%)低于開胸組(38.89 %),差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。VATS組術(shù)后復(fù)發(fā)率(11.11%)低于開胸組(20.37%),差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。 結(jié)論 胸腔鏡治療Ⅰ、Ⅱ期NSCLC療效顯著,術(shù)后生存率高,術(shù)后并發(fā)癥發(fā)生率及復(fù)發(fā)率低,值得臨床應(yīng)用及推廣。

Clinical study of thoracoscopy in the treatment of stage Ⅰ and Ⅱ NSCLC

[Abstract] Objective To study the clinical efficacy of thoracoscopy in the treatment of stage Ⅰ and Ⅱ NSCLC(non-small cell lung cancer). Methods 108 patients with stage Ⅰ and Ⅱ NSCLC who were given thoracic surgery in our hospital from January 2012 to December 2013 were selected as the study subjects. According to the random number table, the patients were divided into VATS group and thoracotomy group with the ratio of 1∶1,with 54 patients in each group. There was no statistically significant difference in the general data between the two groups(P<0.05). VATS group was given VATS,and thoracotomy group was given thoracotomy of lung cancer. The surgical indicators such as surgery time,intraoperative volume of bleeding, number of dissected lymph nodes, postoperative drainage time,and levels of CD3+, CD4+, CD8+ and other immune cells before surgery and 7 days after surgery were observed. The levels of CRP, TNF-α, IL-6 and other inflammatory factors before surgery and 7 days after surgery, as well as survival rate, complications and recurrence within 3 years after the surgery were observed. Results The surgery time, intraoperative volume of bleeding, postoperative drainage time in VATS group were lower than those in thoracotomy group, and the differences were statistically significant(P<0.05). There was no significant difference in the number of dissected lymph nodes between VATS group and thoracotomy group(P>0.05). The level of CD3+ and CD4+ in VATS group was higher than those in thoracotomy group 7 days after surgery, and CD8+ level was lower than that in thoracotomy group(P<0.05). The difference was statistically significant(P<0.05). The levels of CRP, TNF-α and IL-6 in VATS group were lower than those in thoracotomy group 7 days after surgery, and the differences were statistically significant(P<0.05). The survival rates 1-year, 2-year and 3-year after the surgery(94.44%, 81.48%, 56.14%) in VATS group was significantly higher than those in the thoracotomy group(72.22%, 57.41%, 37.03%), and the differences were statistically significant(P<0.05). The incidence rate of postoperative complications(9.26%) in VATS group was lower than that in thoracotomy group(38.89%), and there was statistically significant difference(P<0.05). The recurrence rate(11.11%) in VATS group was lower than that in thoracotomy group(20.37%), and there was statistically significant difference(P<0.05). Conclusion Thoracoscopy in the treatment of stage Ⅰ and Ⅱ NSCLC is effective, with high postoperative survival rate and low incidence rate of postoperative complications and recurrence rate, which is worthy of clinical application and promotion.endprint

NSCLC(非小細(xì)胞肺癌)為我國(guó)發(fā)病率、死亡率較高的惡性腫瘤,資料顯示,約有80%的肺癌患者為NSCLC[2]。對(duì)于NSCLC的治療,過去傳統(tǒng)上臨床多行開胸手術(shù)治療,但開胸手術(shù)創(chuàng)傷大,并發(fā)癥多,不利于患者術(shù)后康復(fù)[3]。近年來,隨著微創(chuàng)技術(shù)逐漸應(yīng)用于臨床,胸腔鏡以其創(chuàng)傷小、并發(fā)癥少、術(shù)后化療耐受水平高等優(yōu)點(diǎn)已逐漸應(yīng)用于臨床[4]。2012年1月~2013年12月,我院胸外科將VATS(全胸腔鏡下肺葉切除術(shù))應(yīng)用于Ⅰ、Ⅱ期NSCLC的臨床治療,其綜合療效較好,現(xiàn)報(bào)道如下。
1 資料與方法
1.1 一般資料
將2012年1月~2013年12月在我院胸外科接受手術(shù)治療的Ⅰ、Ⅱ期NSCLC患者108例作為研究對(duì)象,其中男67例(62.04%),女41例(37.96%),年齡52~78歲,平均(63.15±1.33)歲。病理分期:Ⅰa期患者24例(22.22%),Ⅰb期患者27例(25.00%),Ⅱa期患者24例(22.22%),Ⅱb期患者33例(30.56%)。病理分型:腺癌67例(62.04%),鱗癌33例(30.56%),鱗腺癌6例(5.56%),其他2例(1.85%)。腫瘤部位:左上肺21例(19.44%),左下肺33例(30.56%),右上肺27例(25.00%),右中肺16例(14.81%),右下肺11例(10.19%)。納入標(biāo)準(zhǔn)[1]:病理診斷為NSCLC,且病理分期為Ⅰ、Ⅱ期患者;術(shù)前胸部CT檢查提示病灶直徑≤4 cm患者;Karnofsky評(píng)分>70分患者;手術(shù)適應(yīng)證患者;簽訂知情同意書患者。排除標(biāo)準(zhǔn):腫瘤已轉(zhuǎn)移患者;術(shù)前未接受其他治療患者;術(shù)后病理診斷為腺樣囊性癌、黏液表皮樣癌、類癌等其他肺癌患者;合并其他部位惡性腫瘤患者;難以耐受手術(shù)患者;特征人群(精神疾病患者、神經(jīng)疾病患者、孕婦、哺乳期女性患者)。將所有患者按照隨機(jī)數(shù)字表法1:1分為VATS組與開胸組,各54例,兩組性別、年齡、病理分期、病理分型等一般資料差異無統(tǒng)計(jì)學(xué)意義(P>0.05),具有可比性。見表1。
1.2 方法
1.2.1 VATS組 VATS組行VATS治療。患者取側(cè)臥位,靜脈復(fù)合麻醉,雙腔氣管插管。于第7~8肋間腋中線切出1.0~1.5 cm切口為觀察孔,將胸腔鏡置入胸腔進(jìn)行探查,準(zhǔn)確掌握腫瘤的位置、大小、胸膜粘連及浸潤(rùn)情況。于第4~5肋間腋前線切出4.0~5.0 cm切口為主操作孔,于第6~7肋間腋后線切出1.5~2.0 cm切口為輔助操作孔。胸腔鏡下游離肺靜脈及動(dòng)脈分支,切除肺葉后移除。胸腔鏡下以卵圓鉗清掃腫瘤相應(yīng)部位縱膈淋巴結(jié),常規(guī)放置胸腔引流管后縫合各操作孔。
1.2.2 開胸組 開胸組行開胸根治術(shù)?;颊呷?,側(cè)臥位,雙腔氣管插管。于第4~5肋間后外側(cè)切除15~20 cm切口,撐開器撐開肋骨后進(jìn)入胸腔,將縱膈胸膜剪開,直視狀態(tài)解剖肺靜脈、動(dòng)脈各分支,并將其于縫扎、結(jié)扎后切斷,以結(jié)扎縫合法封閉支氣管斷端,切除肺葉后常規(guī)清理肺門及縱膈淋巴結(jié),常規(guī)放置胸腔引流管后關(guān)胸。
1.3 觀察指標(biāo)
觀察兩組手術(shù)時(shí)間、術(shù)中出血量、清掃淋巴結(jié)個(gè)數(shù)、術(shù)后引流時(shí)間等手術(shù)指標(biāo);術(shù)前及術(shù)后7 d CD3+、CD4+、CD8+等免疫細(xì)胞水平;手術(shù)前及術(shù)后7 d CRP、TNF-α、IL-6等炎性因子水平;3年內(nèi)生存率、并發(fā)癥、復(fù)發(fā)等指標(biāo)。CD3+、CD4+、CD8+等以流式細(xì)胞儀檢測(cè),TNF-α、CRP、IL-6以雙抗體夾心ELISA法檢測(cè)。
1.4 統(tǒng)計(jì)學(xué)處理
采用SPSS19.0統(tǒng)計(jì)學(xué)軟件分析,計(jì)量資料以(x±s)表示,組內(nèi)比較行配對(duì)t檢驗(yàn)、組間比較行獨(dú)立樣本t檢驗(yàn);計(jì)數(shù)資料以例數(shù)、百分比表示,組內(nèi)、組間比較用χ2檢驗(yàn),P<0.05表示差異有統(tǒng)計(jì)學(xué)意義。
2 結(jié)果
2.1 兩組手術(shù)指標(biāo)比較
VATS組手術(shù)時(shí)間、術(shù)中出血量、術(shù)后引流時(shí)間均顯著低于開胸組,兩組比較差異有統(tǒng)計(jì)學(xué)意義(均P<0.05);VATS組與開胸組清掃淋巴結(jié)個(gè)數(shù)差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。見表2。
2.2 兩組術(shù)前與術(shù)后7 d免疫細(xì)胞水平比較
術(shù)前兩組CD3+、CD4+、CD8+水平無統(tǒng)計(jì)學(xué)差異(P>0.05);術(shù)后7 d兩組CD3+、CD4+水平均顯著升高,CD8+水平顯著降低,手術(shù)前后差異有統(tǒng)計(jì)學(xué)意義(P<0.05);術(shù)后7 d VATS組CD3+、CD4+水平顯著高于開胸組,CD8+水平顯著低于開胸組,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。見表3。
2.3 兩組術(shù)前及術(shù)后7 d炎性因子水平比較
術(shù)前兩組CRP、TNF-α、IL-6水平無統(tǒng)計(jì)學(xué)差異(P>0.05);術(shù)后7 d兩組CRP、TNF-α、IL-6水平均顯著升高,組內(nèi)比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05);術(shù)后7 d VATS組CRP、TNF-α、IL-6水平顯著低于開胸組,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。見表4。
2.4 兩組術(shù)后生存率、并發(fā)癥、復(fù)發(fā)等指標(biāo)比較
VATS組術(shù)后1、2、3年生存率分別為94.44%、81.48%、56.14%,開胸組術(shù)后1、2、3年生存率分別為72.22%、57.41%、37.03%,兩組患者術(shù)后1、2、3年生存率比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。VATS組術(shù)后出現(xiàn)心律失常2例,肺部感染3例,術(shù)后并發(fā)癥發(fā)病率為9.26%,開胸組術(shù)后出現(xiàn)心律失常5例,切口脂肪液化3例,肺部感染7例,切口感染6例,術(shù)后并發(fā)癥發(fā)病率為38.89%,兩組術(shù)后并發(fā)癥發(fā)病率有顯著統(tǒng)計(jì)學(xué)差異(P<0.05)。VATS組術(shù)后復(fù)發(fā)率為11.11%,開胸組術(shù)后復(fù)發(fā)率為20.37%,兩組術(shù)后復(fù)發(fā)率比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。見表5。endprint
3 討論
3.1 胸腔鏡治療Ⅰ、Ⅱ期NSCLC符合加速康復(fù)外科學(xué)理念
在Ⅰ、Ⅱ期NSCLC的臨床治療上,傳統(tǒng)上多行開胸肺葉切除術(shù),盡管開胸肺葉切除術(shù)可有效切除病灶,改善患者預(yù)后,但卻存在著較高的并發(fā)癥發(fā)病率,影響患者康復(fù)[5-6]。與開胸術(shù)比較,胸腔鏡手術(shù)創(chuàng)傷小,可有效促進(jìn)患者術(shù)后快速康復(fù),為早期放化療以及其他輔助治療創(chuàng)造了良好的條件[7]。Marty-Ane CH等[8]認(rèn)為,胸腔鏡局部切除術(shù)治療NSCLC既可使患者肺功能得以更多保留,還可以提高患者的耐受性,降低死亡率。王俊等[9]研究發(fā)現(xiàn),胸腔鏡輔助治療術(shù)中過程平穩(wěn),對(duì)胸腔內(nèi)臟器擾動(dòng)小,術(shù)后恢復(fù)快。Wu N等[10]研究結(jié)果表明,胸腔鏡手術(shù)不但淋巴結(jié)清掃效果與開胸手術(shù)相近,其術(shù)中出血量、手術(shù)時(shí)間均低于開胸術(shù)。在本研究中,VATS組淋巴結(jié)清掃數(shù)與開胸組無差異,術(shù)中出血量、手術(shù)時(shí)間均低于開胸組,與Wu N等[10]的研究較為一致。
3.2 胸腔鏡治療Ⅰ、Ⅱ期NSCLC減輕炎癥反應(yīng),對(duì)患者免疫影響較小
胸腔鏡手術(shù)對(duì)非特異性免疫影響相對(duì)較小,術(shù)后免疫水平可快速恢復(fù),有利于殺滅殘余的腫瘤細(xì)胞,降低復(fù)發(fā)率[11]。Ng CS等[12]研究發(fā)現(xiàn),胸腔鏡術(shù)治療NSCLC術(shù)后IL-6、TNF-α顯著高于開胸組。賴添武等[13]認(rèn)為手術(shù)創(chuàng)傷以及感染均會(huì)抑制免疫功能,且手術(shù)創(chuàng)傷及感染程度與免疫功能抑制水平呈顯著的正相關(guān)關(guān)系。李巖[14]比較VTAS與開胸術(shù)對(duì)NSCLC患者免疫功能的影響,結(jié)果表明術(shù)后7 d VTAS組與開胸組CD3+、CD4+水平顯著高于術(shù)前,且VTAS組CD3+、CD4+水平顯著高于開胸組。陳康[15]研究發(fā)現(xiàn),VTAS術(shù)后CRP水平顯著改善,且改善水平顯著優(yōu)于開胸組。在本研究中,開胸組術(shù)后7 d CD3+、CD4+等免疫細(xì)胞水平及炎性因子顯著高于開胸組,CD8+水平顯著低于開胸組,與上述學(xué)者研究結(jié)果較為接近,提示VTAS對(duì)機(jī)體免疫能力的影響相對(duì)較小,更有利于術(shù)后康復(fù)。
3.3 胸腔鏡治療Ⅰ、Ⅱ期NSCLC相對(duì)開胸手術(shù)創(chuàng)傷小
VTAS術(shù)野清晰,可有效避免過多牽拉肋骨,避免損傷肌肉與肋間神經(jīng),術(shù)后疼痛程度較低,故有利于患者早期咳嗽,促使痰液排出,從而有效避免肺部感染、肺不張等術(shù)后并發(fā)癥的發(fā)生[16]。滕洪生等[17]認(rèn)為VTAS分離淋巴結(jié)較為準(zhǔn)確,避免破壞毛細(xì)血管,減少淋巴液過多流失,從而降低胸腔引流量,便于術(shù)后早期拔除引流管,促進(jìn)患者早日康復(fù),符合目前提出的康復(fù)外科學(xué)理念。藏鑫等[18]認(rèn)為,由于VTAS術(shù)中清掃淋巴結(jié)通常行鈍性分離,并以超聲刀輔助,從而有效減少術(shù)后滲出,而開胸術(shù)清掃淋巴結(jié)多行電刀或剪刀清掃,故術(shù)后滲出較多。在本研究中,VTAS組術(shù)后并發(fā)癥發(fā)病率顯著低于開胸組,提示VTAS治療NSCLC更具優(yōu)勢(shì)。
綜上所述,胸腔鏡治療Ⅰ、Ⅱ期NSCLC可明顯減輕患者機(jī)體炎癥反應(yīng),同時(shí)對(duì)患者機(jī)體免疫功能影響小,且手術(shù)切口小、手術(shù)時(shí)間短,術(shù)中出血量少,術(shù)后生存率高,術(shù)后并發(fā)癥發(fā)生率及復(fù)發(fā)率低,符合加速康復(fù)外科學(xué)理念,可有效改善患者預(yù)后等特點(diǎn),值得臨床應(yīng)用及推廣。
