呼吸機(jī)相關(guān)性肺炎患者支氣管肺泡灌洗液sTREM-1的變化及意義

目的 探討呼吸機(jī)相關(guān)性肺炎(VAP)患者支氣管肺泡灌洗液中可溶性髓系細(xì)胞觸發(fā)受體-1(sTREM-1)的動(dòng)態(tài)變化及其臨床意義。 方法 選擇臨床疑診VAP患者112例,通過(guò)微支氣管肺泡灌洗技術(shù)獲取其BALF液體,測(cè)定其BALF中sTREM-1、血清CRP水平及臨床肺部感染積分(CPIS)。動(dòng)態(tài)觀察確診的74例VAP患者在入組第1、4、7、14天(或脫機(jī)/死亡當(dāng)天)BALF中sTREM-1、血清CRP水平及CPIS變化趨勢(shì),并比較存活者(39例)與死亡者(35例)各指標(biāo)的差異。 結(jié)果 VAP組與非VAP組在臨床疑診當(dāng)天BALF中sTREM-1、血清CRP水平及CPIS評(píng)分明顯升高,差異有統(tǒng)計(jì)學(xué)意義(P<0.01)。ROC曲線分析顯示BALF中sTREM-1水平在153.18pg/ml時(shí),診斷VAP有最佳的敏感性和特異性,分別為86.49%和84.21%,ROC曲線下面積為0.882。VAP組中存活者BALF中sTREM-1水平在第7天已經(jīng)下降,第14天或脫機(jī)前已經(jīng)降至非VAP組水平,VAP組死亡者BALF中sTREM-1水平在第7、14天或死亡前持續(xù)增高,與存活者在相同時(shí)間點(diǎn)比較差異有統(tǒng)計(jì)學(xué)意義(P<0.01)。VAP組BALF中sTREM-1水平與APACHEII評(píng)分呈正相關(guān)(r=0.526,P<0.01)。 結(jié)論 BALF中sTREM-1水平在>153.18pg/ml時(shí),有利于早期診斷VAP;VAP患者BALF中sTREM-1水平變化與病情的嚴(yán)重程度及預(yù)后相關(guān)。
可溶性髓系細(xì)胞觸發(fā)受體-1 呼吸機(jī)相關(guān)性肺炎 微支氣管肺泡灌洗
【 Abstract】Objective To investigate the changes of soluble triggering receptor expressed on myeloid cells(sTREM-1)in non-bronchoscope broncho-alveolar lavage fluid (BALF)of patients with ventilator associated pneumonia (VAP)and its clinical significance. Methods One hundred and twelve suspected VAP patients were included in the study,the levels of sTREM-1 in BALF,serum C-reactive protein(CRP)and clinical pulmonary infection scores(CPIS)were determined.The dynamic changes of these parameter were observed in 74 VAP patients at d0,4,7 and 14 of suspected diagnosis,or the day of ventilator weaning or death. Results The levels of BALF sTREM-1,serum CRP and CLIS scores were significant higher in VAP patients compared to non-VAP patients at the day of suspected diagnosis(P<0.01).Receiver operating characteristic(ROC)curve was analyzed, if 153.18pg/ml sTREM-1 in BALF was selected as cut-off point of VAP diagnosis,the sensitivity,specificity and the area of ROC curve was 86.49%,84.21%and 0.882 respectively.In VAP patients,the levels of BALF sTREM-1 were continuously rose in non-survivor group during d1~d14,although those in survivor group began to decrease on d7 after suspected diagnosis of VAP.There was significant difference between non-survivor and survivor group on the levels of BALF sTREM-1 at the same time point.Correlation analysis shows that the levels of BALF sTREM-1 had positive correlation with APACHEII score (r=0.526,P<0.01). Conclusion 153.18pg/ml of BALF sTREM-1 can be used as a cut-off point for diagnosis of VAP.Monitoring the changes of BALF sTREM-1 may be helpful to predict the severity of disease and to make the prognosis.

呼吸機(jī)相關(guān)性肺炎(VAP)是機(jī)械通氣過(guò)程中常見(jiàn)而又嚴(yán)重的并發(fā)癥之一,有研究資料顯示,VAP是重癥監(jiān)護(hù)病房(ICU)發(fā)生的最常見(jiàn)醫(yī)院感染之一,其病死率在33%~71%[1]??扇苄运柘导?xì)胞觸發(fā)受體-1(sTREM-1)是TREM-1的分泌亞型,感染過(guò)程中釋放于血液或體液中,是鑒別感染與非感染疾病的敏感指標(biāo)[2]。本研究擬通過(guò)動(dòng)態(tài)測(cè)定VAP患者支氣管肺泡灌洗液(BALF)中的sTREM-1水平,探討VAP患者氣道局部sTREM-1水平變化及其與炎癥嚴(yán)重程度和預(yù)后的關(guān)系。
1.1 對(duì)象 收集我院ICU2008-01—2010-12收治的機(jī)械通氣患者112例,患者在機(jī)械通氣前無(wú)肺部感染,使用機(jī)械通氣48h后出現(xiàn)發(fā)熱,氣管支氣管內(nèi)分泌物增多、膿性,臨床疑診VAP,排除機(jī)械通氣時(shí)已有肺部感染或機(jī)械通氣時(shí)間≤48h者。其中男68例,女44例,年齡24~85(58.7±13.6)歲。原發(fā)疾病為多發(fā)傷36例,腦血管意外45例,心臟疾病18例,大手術(shù)后12例,電擊傷1例。記錄入選患者的年齡、性別、基礎(chǔ)疾病、APACHEⅡ評(píng)分、臨床肺部感染積分(CPIS)及28d后的轉(zhuǎn)歸(死亡或存活)。根據(jù)有無(wú)發(fā)生VAP,分為VAP組和非VAP組。VAP組74例,男45例,女29例,年齡28~85(60.15± 14.31)歲;死亡35例,病死率47.30%。非VAP組38例,男23例,女15例,年齡24~82(57.35±13.59)歲;死亡12例,病死率31.58%。VAP組患者APACHEⅡ評(píng)分為(20.95±4.94)分,非VAP組為(19.45±3.45)分,兩組年齡、性別及疾病嚴(yán)重程度差異均無(wú)統(tǒng)計(jì)學(xué)意義(均P>0.05)。
1.2 方法 入選的機(jī)械通氣患者在疑診VAP當(dāng)天通過(guò)微支氣管肺泡灌洗技術(shù)獲取其BALF液體標(biāo)本兩份,一份送細(xì)菌學(xué)培養(yǎng),一份高速離心后取上清液,-20°冰箱保存,用于測(cè)定sTREM-1水平。同時(shí)留取患者空腹靜脈血5ml,經(jīng)高速離心后分離血清,置-20℃冰箱保存,用于測(cè)定血常規(guī)、CRP等。VAP確診方法:機(jī)械通氣48h后通過(guò)微支氣管肺泡灌洗技術(shù)獲取其BALF,培養(yǎng)出細(xì)菌,菌落技術(shù)>104CFU/ml,并符合下列條件2項(xiàng)或以上者:(1)X線顯示新發(fā)生的或進(jìn)展性的肺浸潤(rùn);(2)體溫>38℃或比平時(shí)體溫上升≥1℃;(3)血白細(xì)胞計(jì)數(shù)>10×109/L;或比基礎(chǔ)白細(xì)胞增高25%以上;(4)氣管支氣管內(nèi)出現(xiàn)膿性分泌物。由兩位專(zhuān)家根據(jù)臨床及微生物培養(yǎng)結(jié)果來(lái)確診。對(duì)確診的VAP患者在入組第3、7、14天(或脫機(jī)/死亡當(dāng)天)采用同樣的方法檢測(cè)sTREM-1濃度。微支氣管肺泡灌洗技術(shù)獲取BALF標(biāo)本采用Gibot的方法[3],通過(guò)非纖維支氣管鏡的方法采集。
1.3 觀察指標(biāo) 采用ELSIA法檢測(cè)BALF中sTREM-1水平,試劑盒均購(gòu)自美國(guó)R&D公司,操作按試劑盒說(shuō)明書(shū)進(jìn)行。采用透射免疫比濁法檢測(cè)血清CRP水平。
1.4 統(tǒng)計(jì)學(xué)處理 采用SPSS16.0統(tǒng)計(jì)軟件,計(jì)量資料以 表示,組間比較采用t檢驗(yàn);繪制ROC曲線,比較上述各指標(biāo)的敏感性、特異性等,并對(duì)部分?jǐn)?shù)據(jù)之間的關(guān)系進(jìn)行Pearson相關(guān)性分析。
2.1 兩組患者擬診當(dāng)天sTREM-1水平、血清CRP及CPIS評(píng)分比較 見(jiàn)表1。

由表1可見(jiàn),VAP組sTREM-1、血清CRP水平及CPIS評(píng)分明顯高于非VAP組,差異均有統(tǒng)計(jì)學(xué)意義(均P<0.01)。
2.2 sTREM-1水平與血清CRP、CPIS對(duì)早期診斷VAP的診斷價(jià)值比較 擬診當(dāng)天,BALF中sTREM-1cutoff值為153.18pg/ml時(shí),診斷VAP的敏感性86.49%,特異性84.21%,ROC曲線下面積為0.882。血清CRP cutoff值為43.97mg/L對(duì)診斷VAP的敏感性64.86%,特異性71.05%,ROC曲線下面積為0.723;CPIS≥5分時(shí),診斷VAP的敏感性47.3%,特異性86.8%,ROC曲線下面積為0.823。BALF中sTREM-1診斷VAP的ROC曲線見(jiàn)圖1。

2.3 VAP組存活者與死亡者BALF中sTREM-1水平比較 見(jiàn)表2。
由表2可見(jiàn),死亡者sTREM-1水平持續(xù)增高,存活組在第7天已經(jīng)下降,第14天或脫機(jī)前已經(jīng)降到非VAP患者水平,差異均有統(tǒng)計(jì)學(xué)意義(均P<0.01)。
2.4 VAP患者sTREM-1水平與APACHEⅡ評(píng)分相關(guān)性分析 sTREM-1水平與APACHEⅡ評(píng)分呈正相關(guān)(r= 0.526,P<0.01)。

髓細(xì)胞觸發(fā)受體(TREM)是Bouchon等[4]于2000年首次發(fā)現(xiàn)的一組表達(dá)于髓系細(xì)胞的觸發(fā)受體,其中TREM-1選擇性地表達(dá)于中性粒細(xì)胞和一部分單核細(xì)胞(CD14+)。TREM-1有促進(jìn)炎性因子分泌的作用,能誘導(dǎo)中性粒細(xì)胞和單核細(xì)胞分泌TNF-α、IL-1β、IFN-γ等促炎因子,誘導(dǎo)中性粒細(xì)胞釋放髓過(guò)氧化物酶;在細(xì)胞內(nèi)部,TREM-1可誘發(fā)鈣離子轉(zhuǎn)移,并使細(xì)胞表面信號(hào)相關(guān)激酶1,2和磷脂酶C酪氨酸磷酸化,最終可引起炎癥反應(yīng)的增強(qiáng)、放大,導(dǎo)致“過(guò)度”的炎癥反應(yīng)[5]。sTREM-1是TREM-1的可溶形式,有觀點(diǎn)認(rèn)為sTREM-1是TREM-1的mRNA剪接突變體編碼的一種缺乏跨膜結(jié)構(gòu)域的分泌亞型,感染過(guò)程中sTREM-1可以釋放入體液且與感染嚴(yán)重程度密切相關(guān)[2]。Meta分析發(fā)現(xiàn),sTREM-1診斷細(xì)菌感染的敏感度為82%,特異度為86%,是預(yù)測(cè)感染的一個(gè)精確指標(biāo)[6]。本研究顯示,VAP患者BALF中的sTREM-1水平明顯高于非VAP患者,與常規(guī)的炎癥指標(biāo)CRP和臨床評(píng)分指標(biāo)CPIS相比,經(jīng)微支氣管肺泡灌洗技術(shù)獲取sTREM-1濃度達(dá)153.18pg/ml,作為合理的診斷濃度閾值,可得到更好的敏感性及特異性,分別為86.49%及84.21%,與Huh等[7]經(jīng)纖維支氣管鏡行支氣管肺泡灌洗研究所得出的184pg/ml的診斷閾值相似,但我們通過(guò)微支氣管肺泡灌洗技術(shù)獲取BALF標(biāo)本,相對(duì)通過(guò)纖維支氣管鏡獲得BALF技術(shù)而言,創(chuàng)傷小,操作方便,可由護(hù)士操作完成,更具有實(shí)用性。
近年來(lái)研究發(fā)現(xiàn)膿毒癥患者血漿sTREM-1水平明顯升高,并成為幫助判斷膿毒癥患者預(yù)后的一個(gè)獨(dú)立指標(biāo)[8-9]。也有研究發(fā)現(xiàn)重癥肺炎患者誘導(dǎo)痰中sTREM-1、TNF-α及IL-10水平顯著高于正常對(duì)照組,且隨病情加重而上升,隨病情好轉(zhuǎn)而下降,提示sTREM-1可能是一種促炎介質(zhì),其變化趨勢(shì)能反映病情轉(zhuǎn)歸[10]。但探討B(tài)ALF中sTREM-1水平與VAP疾病嚴(yán)重程度及預(yù)后相關(guān)性的研究不多。本研究顯示,VAP患者BALF中 sTREM-1水平與APACHEⅡ評(píng)分呈正相關(guān),同時(shí)VAP存活者第7天sTREM-1水平開(kāi)始下降,到第14天或脫機(jī)前已經(jīng)降至非VAP水平,而死亡組BALF中sTREM-1水平持續(xù)維持高水平,與Wu[11]報(bào)道相似。這也表明動(dòng)態(tài)觀察BALF中sTREM-1水平可以幫助判斷VAP患者的病情及預(yù)測(cè)患者預(yù)后。
綜上所述,通過(guò)本研究,我們發(fā)現(xiàn)經(jīng)微支氣管肺泡灌洗技術(shù)獲得BALF中sTREM-1水平作為VAP的診斷指標(biāo),敏感性、特異性均較高,且操作方便、創(chuàng)傷小,快速得到結(jié)果,有利于VAP的早期診斷。同時(shí)動(dòng)態(tài)檢測(cè)BALF中sTREM-1水平能反映VAP患者的疾病嚴(yán)重程度,幫助判斷預(yù)后。
